Staging: b3dfg: fixups and improvements
- Added support for cable plug/unplug detection.
- Improvements to error handling.
- Switch to the pci_* DMA API.
- Removed set_num_buffers functionality.
- Locking review.
- Unconditionally disable transmission when releasing device.
