Pull SCSI updates from James Bottomley:

Currently, the code does:

}
Zeroing bits 0 and 1 of a variable that is 0 is not necessary. So directly
set the variable to 1.

Rename .slave_alloc() into .sdev_init() and .slave_destroy() into
.sdev_destroy(). The new names make it clear that these are actions on
SCSI devices. Make this change in the SCSI core, SCSI drivers and also
in the ATA drivers. No functionality has been changed.
This patch has been created as follows:
* Change the text "slave_alloc" into "sdev_init" in all source files
except those in drivers/net/ and Documentation/.
* Change the text "slave_destroy" into "sdev_destroy" in all source
files except those in drivers/net/ and Documentation/.
* Rename lpfc_no_slave() into lpfc_no_sdev().
* Manually adjust whitespace where necessary to restore vertical
alignment (dc395x driver and include/linux/libata.h).

The workqueue maintainer wants to remove the create*_workqueue() macros
because these macros always set the WQ_MEM_RECLAIM flag and because these
only support literal workqueue names. Hence this patch that replaces the
create*_workqueue() invocations with the definition of this macro. The
WQ_MEM_RECLAIM flag has been retained because I think that flag is necessary
for workqueues created by storage drivers. This patch has been generated by
running spatch and git clang-format. spatch has been invoked as follows:

Some firmware versions of the 9600 series SAS HBA byte-swap the REPORT
ZONES command reply buffer from ATA-ZAC devices by directly accessing the
buffer in the host memory. This does not respect the default command DMA
direction and causes IOMMU page faults on architectures with an IOMMU
enforcing write-only mappings for DMA_FROM_DEVICE DMA driection (e.g. AMD
hosts).

Avoid such issue by always mapping the buffer of REPORT ZONES commands
using DMA_BIDIRECTIONAL (read+write IOMMU mapping). This is done by
introducing the helper function _base_scsi_dma_map() and using this helper
in _base_build_sg_scmd() and _base_build_sg_scmd_ieee() instead of calling
directly scsi_dma_map().

Setting QUEUE_FLAG_NOMERGES was added in commit d1b01d14b7ba ("scsi:
mpt3sas: Set NVMe device queue depth as 128") without any explanation.
Drivers should second guess the block layer merge decisions, so remove
the flag.

The function mpi3mr_qcmd() of the mpi3mr driver is able to indicate to
the HBA if a read or write command directed at an ATA device should be
translated to an NCQ read/write command with the high prioiryt bit set
when the request uses the RT priority class and the user has enabled NCQ
priority through sysfs.
However, unlike the mpt3sas driver, the mpi3mr driver does not define
the sas_ncq_prio_supported and sas_ncq_prio_enable sysfs attributes, so
the ncq_prio_enable field of struct mpi3mr_sdev_priv_data is never
actually set and NCQ Priority cannot ever be used.
Fix this by defining these missing atributes to allow a user to check if
an ATA device supports NCQ priority and to enable/disable the use of NCQ
priority. To do this, lift the function scsih_ncq_prio_supp() out of the
mpt3sas driver and make it the generic SCSI SAS transport function
sas_ata_ncq_prio_supported(). Nothing in that function is hardware
specific, so this function can be used in both the mpt3sas driver and
the mpi3mr driver.

There is a potential out-of-bounds access when using test_bit() on a single
word. The test_bit() and set_bit() functions operate on long values, and
when testing or setting a single word, they can exceed the word
boundary. KASAN detects this issue and produces a dump:

The prior strscpy() replacement of strncpy() here expected the
manufacture_reply strings to be NUL-terminated, but it is possible
they are not, as the code pattern here shows, e.g., edev->vendor_id
being exactly 1 character larger than manufacture_reply->vendor_id,
and the replaced strncpy() was copying only up to the size of the
source character array. Replace this with memtostr(), which is the
unambiguous way to convert a maybe not-NUL-terminated character array
into a NUL-terminated string.

strncpy() is deprecated for use on NUL-terminated destination strings
[1] and as such we should prefer more robust and less ambiguous string

The replacement in mpt3sas_base.c is a trivial one because desc is already
zero-initialized meaning there is no functional change here.
For mpt3sas_transport.c, we know edev is zero-initialized as well while
manufacture_reply comes from dma_alloc_coherent(). No functional change
here either.
For all cases, use the more idiomatic strscpy() usage of: strscpy(dest,
src, sizeof(dest))

If the driver detects that the controller is not ready before sending the
first IOC facts command, it will wait for a maximum of 10 seconds for it to
become ready. However, even if the controller becomes ready within 10
seconds, the driver will still issue a diagnostic reset.
Modify the driver to avoid sending a diag reset if the controller becomes
ready within the 10-second wait time.

Add a new IOCTL command MPT3ENABLEDIAGSBRRELOAD. As a part of firmware
update operation, applications use this IOCTL command to set the SBR reload
bit in the Host Diagnostic register. This permits HBA firmware to be
updated without powercycling the system.

The mpt3sas_ctl_exit() should be called after communication with the
controller stops but currently it may cause false warnings about not
released memory. Fix this by letting mpt3sas_ctl_exit() handle misc driver
release per driver and release DMA in mpt3sas_ctl_release() per ioc.

mpt3sas_scsih.c:_scsih_scan_for_devices_after_reset() allocates and fetches
a MPI2_CONFIG_PAGE_RAID_VOL_0 struct (Mpi2RaidVolPage0_t) and a
MPI2_CONFIG_PAGE_RAID_VOL_1 struct (Mpi2RaidVolPage1_t), but does not
include the terminal flexible array members in the struct size
calculations, fetch those members, or otherwise use those members in any
way.
These dynamic allocations can be replaced with local variables.

The per-adapter struct (struct MPT3SAS_ADAPTER) contains a
MPI2_CONFIG_PAGE_IO_UNIT_8 (Mpi2IOUnitPage8_t) iounit_pg8 member that is
populated by mpt3sas_base.c:_base_static_config_pages().
As the name of that function indicates, the iounit_pg8 member represents a
static configuration page data structure that rarely changes, and is among
several such static config pages that are currently being fetched once per
adapter per init (or reset) and copied to the per-adapter struct for later
use.
However, unlike the other static config pages, the iounit_pg8 member is
never actually used outside of _base_static_config_pages(). Also,
Mpi2IOUnitPage8_t has a flexible array member, making its presence in the
_middle_ of the per-adapter struct rather strange.
Remove this member from the per-adapter struct and fix up the portion of
_base_static_config_pages() that uses it.

flexible array member.
As all users of MPI26_CONFIG_PAGE_PIOUNIT_1 (Mpi26PCIeIOUnitPage1_t) do not
use PhyData[], no further source changes are required to accommodate its
reduced sizeof():
- mpt3sas_config.c:mpt3sas_config_get_pcie_iounit_pg1() fetches a
Mpi26PCIeIOUnitPage1_t into a caller-provided buffer, and may fetch
and write PhyData[] into that buffer depending on its sz argument.
It has one caller:
- mpt3sas_base.c:_base_assign_fw_reported_qd() passes
sizeof(Mpi26PCIeIOUnitPage1_t) as sz, but does not use PhyData[].

As all users of MPI2_CONFIG_PAGE_IO_UNIT_8 (Mpi2IOUnitPage8_t) do not use
Sensor[], no further source changes are required to accommodate its reduced
sizeof():
- mpt3sas_config.c:mpt3sas_config_get_iounit_pg8() fetches a
Mpi2IOUnitPage8_t into a caller-provided buffer, assuming
sizeof(Mpi2IOUnitPage8_t) as the buffer size. It has one caller:
- mpt3sas_base.c:_base_static_config_pages() passes the address of the
Mpi2IOUnitPage8_t iounit_pg8 member of the per-adapter struct (struct
MPT3SAS_ADAPTER *ioc) as the buffer. The assumed buffer size is
therefore correct.
However, the only subsequent use in mpt3sas of the thus populated
ioc->iounit_pg8 is a little further on in the same function, and this
use does not involve ioc->iounit_pg8.Sensor[].
Note that iounit_pg8 occurs in the middle of the per-adapter struct,
not at the end. The per-adapter struct is extensively used throughout
mpt3sas even if its iounit_pg8 member isn't, resulting in an
especially large amount of noise when comparing binary changes
attributable to this commit.

These terminal 1-length variable arrays can be directly converted into C99
flexible array members without any binary changes.
In most cases, they belong to unused structs, or to structs used only by
unused code. The remaining few coincidentally have their sizes calculated
in roundabout ways that do not depend on the sizeof() their structs.

Pull SCSI updates from James Bottomley:
"Updates to the usual drivers (ufs, megaraid_sas, lpfc, target, ibmvfc,
scsi_debug) plus the usual assorted minor fixes and updates.
The major change this time around is a prep patch for rethreading of
the driver reset handler API not to take a scsi_cmd structure which
starts to reduce various drivers' dependence on scsi_cmd in error
handling"

The retry loop continues to iterate until the count reaches 30, even after
receiving the correct value. Exit loop when a non-zero value is read.
