This makes it possible to hook the device into a more complex board and
ensures it will continue to work with non-DAPM support removed from the
core.

CONFIG_HOTPLUG is going away as an option. As result the __dev*
markings will be going away.
Remove use of __devinit, __devexit_p, __devinitdata, __devinitconst,
and __devexit.

handling and code cleanup simpler.

The Mute Switch should be called Playback Switch to match the volume
and the regmap core has been updated so we don't need to mark all the
registers as non-volatile.

Add a driver supporting the volume control and the mute pin. Shdn pin
and DAPM are not taken care of yet.
