Rename all USB drivers with their own directory under
drivers/media/video into drivers/media/usb and update the
building system.

This prevents checkpatch warnings generated when defining
'static const char *foo[]' arrays. It makes sense to use
const char * const * anyway since the pointers in the array
are indeed const.

We dereference "maskptr" unconditionally at the start of the function
and also inside the call to parse_tlist() towards the end of the
function. This function is called from store_val_any() and it always
passes a non-NULL pointer.

default value
The pvrusb2 control mechanism up until now has used a constant int to
hold a control's default value. This change makes it possible to
retrieve the control's default through some other means, e.g. as a
result of a query from lower level software.

When an enumeration control is changed, the pvrusb2 driver assumed
that the enumeration values were continuous. That is no longer true;
this change allows for properly input validation even when not all
enumeration values are legal (which can happen with input selection
based on what the hardware supports).

In the previous patch we exploited the get_{min,max}_value facility to adjust
min/max allowable frequencies on the fly, depending on tuner mode.
Unfortunately, this facility was not used inside the *sym_to_val() function
that translates what we echo to sysfs, which means we got an -ERANGE despite
asking for a frequency between what we read to be min/max.
This patch corrects this small omission.

The internal control implementation in the pvrusb2 driver normally
encodes integer range limits using literal values in a const
structure. This change adds two function pointers, which if not null
will be called through in order to determine integer min / max
values.

Implement V4L2 driver for the Hauppauge PVR USB2 TV tuner.
The Hauppauge PVR USB2 is a USB connected TV tuner with an embedded
cx23416 hardware MPEG2 encoder. There are two major variants of this
device; this driver handles both. Any V4L2 application which
understands MPEG2 video stream data should be able to work with this
device.
