[ Upstream commit 2ef422f063b74adcc4a4a9004b0a87bb55e0a836 ]
In the unlikely event that workqueue allocation fails and returns NULL in
mlx5_mkey_cache_init(), delete the call to
mlx5r_umr_resource_cleanup() (which frees the QP) in
mlx5_ib_stage_post_ib_reg_umr_init(). This will avoid attempted double
free of the same QP when __mlx5_ib_add() does its cleanup.

ucmd in hns_roce_create_qp_common() are not initialized. But it works fine
until new member sdb_addr is added to struct hns_roce_ib_create_qp.
If the user-mode driver uses an old version ABI, then the value of the new
member will be undefined after ib_copy_from_udata().
This patch fixes it by initialize this variable to 0. And the default value
of the new member sdb_addr will be 0 which is invalid.

[ Upstream commit 9faef73ef4f6666b97e04d99734ac09251098185 ]
The current driver will print all asynchronous events. Some of the
print levels are set improperly, e.g. SRQ limit reach and SRQ last
wqe reach, which may also occur during normal operation of the software.
Currently, the information of these event is printed as a warning,
which causes a large amount of printing even during normal use of the
application. As a result, the service performance deteriorates.
This patch fixes the printing storms by modifying the print level.

Previously when we had a RAW QP, we bound a counter to it when it moved
to INIT state, using the counter context inside RQC.
But when we try to modify that counter later in RTS state we used
modify QP which tries to change the counter inside QPC instead of RQC.
Now we correctly modify the counter set_id inside of RQC instead of QPC
for the RAW QP.

commit 53a3f777049771496f791504e7dc8ef017cba590 upstream.
In case immediate MPA request processing fails, the newly
created endpoint unlinks the listening endpoint and is
ready to be dropped. This special case was not handled
correctly by the code handling the later TCP socket close,
causing a NULL dereference crash in siw_cm_work_handler()
when dereferencing a NULL listener. We now also cancel
the useless MPA timeout, if immediate MPA request
processing fails.
This patch furthermore simplifies MPA processing in general:
Scheduling a useless TCP socket read in sk_data_ready() upcall
is now surpressed, if the socket is already moved out of
TCP_ESTABLISHED state.

commit e193b7955dfad68035b983a0011f4ef3590c85eb upstream.
After scmd_eh_abort_handler() has called the SCSI LLD eh_abort_handler
callback, it performs one of the following actions:
* Call scsi_queue_insert().
* Call scsi_finish_command().
* Call scsi_eh_scmd_add().
Hence, SCSI abort handlers must not call scsi_done(). Otherwise all
the above actions would trigger a use-after-free. Hence remove the
scsi_done() call from srp_abort(). Keep the srp_free_req() call
before returning SUCCESS because we may not see the command again if
SUCCESS is returned.

When trying to destroy QP or CQ, we first decrease the refcount and
potentially free memory regions allocated for the object and then
request the device to destroy the object. If the device fails, the
object isn't fully destroyed so the user/IB core can try to destroy the
object again which will lead to underflow when trying to decrease an
already zeroed refcount.
Deallocate resources in reverse order of allocating them to safely free
them.

We need to call siw_cep_put twice since cep->kref is increased once after
it was initialized.
2. If siw_cm_queue_work can't find a work, which means siw_cep_get is not
called in siw_cm_queue_work, so cep->kref is increased twice by siw_cep_get
and when associate socket with cep after it was initialized. So we need to
call siw_cep_put three times (one in siw_socket_disassoc).
3. siw_send_mpareqrep returns error, this scenario is similar as 2.
So we need to remove one siw_cep_put in the error path.

Currently irdma allows zero-length STAGs to be programmed in HW during
the kernel mode fast register flow. Zero-length MR or STAG registration
disable HW memory length checks.
Improve gaps in bounds checking in irdma by preventing zero-length STAG or
MR registrations except if the IB_PD_UNSAFE_GLOBAL_RKEY is set.
This addresses the disclosure CVE-2023-25775.

[ Upstream commit 706efac4477cdb8be857f6322457de524acc02ff ]
Currently, direct wqe is not supported for wr-list. RoCE driver excludes
direct wqe for wr-list by judging whether the number of wr is 1.
For a wr-list where the second wr is a length-error atomic wr, the
post-send driver handles the first wr and adds 1 to the wr number counter
firstly. While handling the second wr, the driver finds out a length error
and terminates the wr handle process, remaining the counter at 1. This
causes the driver mistakenly judges there is only 1 wr and thus enters
the direct wqe process, carrying the current length-error atomic wqe.
This patch fixes the error by adding a judgement whether the current wr
is a bad wr. If so, use the normal doorbell process but not direct wqe
despite the wr number is 1.

One-element and zero-length arrays are deprecated. So, replace
one-element array in struct irdma_qvlist_info with flexible-array
member.
A patch for this was sent a while ago[1]. However, it seems that, at
the time, the changes were partially folded[2][3], and the actual
flexible-array transformation was omitted. This patch fixes that.
The only binary difference seen before/after changes is shown below:

while folding the patch I've mentioned above.
Worth mentioning is the fact that with this change we save 12 bytes

that:

[ Upstream commit 5d122db2ff80cd2aed4dcd630befb56b51ddf947 ]
If a send packet is dropped by the IP layer in rxe_requester()
the call to rxe_xmit_packet() can fail with err == -EAGAIN.
To recover, the state of the wqe is restored to the state before
the packet was sent so it can be resent. However, the routines
that save and restore the state miss a significnt part of the
variable state in the wqe, the dma struct which is used to process
through the sge table. And, the state is not saved before the packet
is built which modifies the dma struct.
Under heavy stress testing with many QPs on a fast node sending
large messages to a slow node dropped packets are observed and
the resent packets are corrupted because the dma struct was not
restored. This patch fixes this behavior and allows the test cases
to succeed.

Although the code for residual handling in the SRP initiator follows the
SCSI documentation, that documentation has never been correct. Because
scsi_finish_command() starts from the data buffer length and subtracts the

occurs. Hence remove the scsi_set_resid() calls from the SRP initiator if a

LOOPBACK and NONE (tunnel) devices have all-zero MAC addresses.
Currently, siw_device_create() falls back to copying the IB device's
name in those cases, because an all-zero MAC address breaks the RDMA
core address resolution mechanism.
However, at the point when siw_device_create() constructs a GID, the
ib_device::name field is uninitialized, leaving the MAC address to
remain in an all-zero state.
Fabricate a random artificial GID for such devices, and ensure this
artificial GID is returned for all device query operations.

Previously when destroying a QP/RQ, the result of the firmware
destruction function was ignored and upper layers weren't informed
about the failure.
Which in turn could lead to various problems since when upper layer
isn't aware of the failure it continues its operation thinking that the
related QP/RQ was successfully destroyed while it actually wasn't,
which could lead to the below kernel WARN.
Currently, we return the correct firmware destruction status to upper
layers which in case of the RQ would be mlx5_ib_destroy_wq() which
was already capable of handling RQ destruction failure or in case of
a QP to destroy_qp_common(), which now would actually warn upon qp
destruction failure.

commit 186b169cf1e4be85aa212a893ea783a543400979 upstream.
Fixing the ODP registration flow to set the iova correctly.
The calculation in ib_umem_num_dma_blocks() function assumes the iova of
the umem is set correctly.
When iova is not set, the calculation in ib_umem_num_dma_blocks() is
equivalent to length/page_size, which is true only when memory is aligned.
For unaligned memory, iova must be set for the ALIGN() in the
ib_umem_num_dma_blocks() to take effect and return a correct value.
mlx5_ib uses ib_umem_num_dma_blocks() to decide the mkey size to use for
the MR. Without this fix, when registering unaligned ODP MR, a wrong
size mkey might be chosen and this might cause the UMR to fail.
UMR would fail over insufficient size to update the mkey translation:

HW may generate completions that indicates QP is destroyed.
Driver should not be scheduling any more completion handlers
for this QP, after the QP is destroyed. Since CQs are active
during the QP destroy, driver may still schedule completion
handlers. This can cause a race where the destroy_cq and poll_cq
running simultaneously.
Snippet of kernel panic while doing bnxt_re driver load unload in loop.
This indicates a poll after the CQ is freed.

To avoid this, complete all completion handlers before returning the

this will prevent any race mentioned.

introduced a new struct mthca_sqp which doesn't contain struct mthca_qp

to crashes, because mthca_poll_one() expects a qp pointer. Fix this

[ Upstream commit 0af91306e17ef3d18e5f100aa58aa787869118af ]
Driver is not handling the wraparound of the mbox producer index correctly.
Currently the wraparound happens once u32 max is reached.
Bit 31 of the producer index register is special and should be set
only once for the first command. Because the producer index overflow
setting bit31 after a long time, FW goes to initialization sequence
and this causes FW hang.
Fix is to wraparound the mbox producer index once it reaches u16 max.

[ Upstream commit 425e1c9018fdf25cb4531606cc92d9d01a55534f ]
The subroutine rxe_check_bind_mw() in rxe_mw.c performs checks on the mw
access flags before they are set so they always succeed. This patch
instead checks the access flags passed in the send wqe.
