mips_cm_error_report() extracts the cause and other cause from the error
register using shifts. This works fine for the former, as it is stored
in the top bits, and the shift will thus remove all non-related bits.
However, the latter is stored in the bottom bits, hence thus needs masking
to get rid of non-related bits. Without such masking, using it as an
index into the cm2_causes[] array will lead to an out-of-bounds access,
probably causing a crash.
Fix this by using FIELD_GET() instead. Bite the bullet and convert all
MIPS CM handling to the bitfield API, to improve readability and safety.

According to the MIPS32 InterAptiv software manual error codes 24 - 26
of CM2 indicate L2 ECC/parity error with switching to a corresponding
errors info fields. This patch provides these errors parsing code,
which handles the read/write uncorrectable and correctable ECC/parity
errors, and prints instruction causing the fault, RAM array type, cache
way/dword and syndrome associated with the faulty data.

Commit 3885c2b463f6 ("MIPS: CM: Add support for reporting CM cache
errors") adds cm2_causes[] array with map of error type ID and
pointers to the short description string. There is a mistake in
the table, since according to MIPS32 manual CM2_ERROR_TYPE = {17,18}
correspond to INTVN_WR_ERR and INTVN_RD_ERR, while the table
claims they have {0x17,0x18} codes. This is obviously hex-dec
copy-paste bug. Moreover codes {0x18 - 0x1a} indicate L2 ECC errors.

Pull MIPS updates from Paul Burton:
- Support for the MIPSr6 MemoryMapID register & Global INValidate TLB
(GINVT) instructions, allowing for more efficient TLB maintenance
when running on a CPU such as the I6500 that supports these.
- Enable huge page support for MIPS64r6.
- Optimize post-DMA cache sync by removing that code entirely for
kernel configurations in which we know it won't be needed.
- The number of pages allocated for interrupt stacks is now calculated
correctly, where before we would wastefully allocate too much memory
in some configurations.
- The ath79 platform migrates to devicetree.
- The bcm47xx platform sees fixes for the Buffalo WHR-G54S board.
- The ingenic/jz4740 platform gains support for appended devicetrees.

mips_cm_error_report() contains a function call that's incorrectly
indented a level further than it ought to be. Remove a tab from the
start of both affected lines.

The GCR_ERROR_CAUSE.ERR_TYPE field and the GCR_ERROR_MULT.ERR_TYPE
fields can be cleared by either a reset or by writing the current
value of GCR_ERROR_CAUSE.ERR_TYPE to the
GCR_ERROR_CAUSE.ERR_TYPE register.

Do exactly this. Original value of cm_error may be safely written back;
it clears error cause and keeps other bits untouched.

Since commit 68923cdc2eb3 ("MIPS: CM: Add cluster & block args to
mips_cm_lock_other()"), mips_smp_send_ipi_mask() has used
mips_cm_lock_other_cpu() with each CPU number, rather than
mips_cm_lock_other() with the first VPE in each core. Prior to r6,
multicore multithreaded systems such as dual-core dual-thread
interAptivs with CPU Idle enabled (e.g. MIPS Creator Ci40) results in
mips_cm_lock_other() repeatedly hitting WARN_ON(vp != 0).
There doesn't appear to be anything fundamentally wrong about passing a
non-zero VP/VPE number, even if it is a core's region that is locked
into the other region before r6, so remove that particular WARN_ON().

MIPS will soon not be a part of Imagination Technologies, and as such
many @imgtec.com email addresses will no longer be valid. This patch
updates the addresses for those who:
- Have 10 or more patches in mainline authored using an @imgtec.com
email address, or any patches dated within the past year.
- Are still with Imagination but leaving as part of the MIPS business
unit, as determined from an internal email address list.
- Haven't already updated their email address (ie. JamesH) or expressed
a desire to be excluded (ie. Maciej).
- Acked v2 or earlier of this patch, which leaves Deng-Cheng, Matt &
myself.
New addresses are of the form firstname.lastname@mips.com, and all
verified against an internal email address list. An entry is added to
.mailmap for each person such that get_maintainer.pl will report the new
addresses rather than @imgtec.com addresses which will soon be dead.
Instances of the affected addresses throughout the tree are then
mechanically replaced with the new @mips.com address.

With Coherence Manager (CM) 3.5 information about the topology of the
system, which has previously only been available through & accessed from
the CM, is now also provided by the Cluster Power Controller (CPC). This
includes a new CPC_CONFIG register mirroring GCR_CONFIG, and similarly a
new CPC_Cx_CONFIG register mirroring GCR_Cx_CONFIG.
In preparation for adjusting functions such as mips_cm_numcores(), which
have previously only needed to access the CM, to also access the CPC
this patch modifies the way we use the various CPS headers. Rather than
having users include asm/mips-cm.h or asm/mips-cpc.h individually we
instead have users include asm/mips-cps.h which in turn includes
asm/mips-cm.h & asm/mips-cpc.h. This means that users will gain access
to both CM & CPC registers by including one header, and most importantly
it makes asm/mips-cps.h an ideal location for helper functions which
need to access the various components of the CPS.

With CM >= 3.5 we have the notion of multiple clusters & can access
their CM, CPC & GIC registers via the apporpriate redirect/other
register blocks. In order to allow for this introduce cluster & block
arguments to mips_cm_lock_other() which configures the redirect/other
region to point at the appropriate cluster, core, VP & register block.
Since we now have 4 arguments to mips_cm_lock_other() & a common use is
likely to be to target the cluster, core & VP corresponding to a
particular Linux CPU number we also add a new mips_cm_lock_other_cpu()
helper function which handles that without the caller needing to
manually pull out the cluster, core & VP numbers.

We currently have fields in struct cpuinfo_mips for the core & VP(E) ID
of a particular CPU, and various pieces of code directly access those
fields. This patch abstracts such access by introducing accessor
functions cpu_core(), cpu_set_core(), cpu_vpe_id() & cpu_set_vpe_id()
and having code that needs to access these values call those functions
rather than directly accessing the struct cpuinfo_mips fields. This
prepares us for changes to the way in which those values are stored in
later patches.
The cpu_vpe_id() function is introduced even though we already had a
cpu_vpe_id() macro for a couple of reasons:
1) It's more consistent with the core, and future cluster, accessors.
2) It ensures a sensible return type without explicit casts.
3) It's generally preferable to use functions rather than macros.

There's no reason for us not to use BIT() & GENMASK() in asm/mips-cm.h
when declaring macros corresponding to register fields. This patch
modifies our definitions to do so.
The *_SHF definitions are removed entirely - they duplicate information
found in the masks, are infrequently used & can be replaced with use of
__ffs() where needed.
The *_MSK definitions then lose their _MSK suffix which is now somewhat
redundant, and users are modified to match.
The field definitions are moved to follow the appropriate register's
accessor functions, which helps to keep the field definitions in order &
to find the appropriate fields for a given register. Whilst here a
comment is added describing each register & including its name, which is
helpful both for linking the register back to hardware documentation &
for grepping purposes.
This also cleans up a couple of issues that became obvious as a result
of making the changes described above:
- We previously had definitions for GCR_Cx_RESET_EXT_BASE & a phony
copy of that named GCR_RESET_EXT_BASE - a register which does not
exist. The bad definitions were added by commit 497e803ebf98 ("MIPS:
smp-cps: Ensure secondary cores start with EVA disabled") and made
use of from boot_core(), which is now modified to use the
GCR_Cx_RESET_EXT_BASE definitions.
- We had a typo in CM_GCR_ERROR_CAUSE_ERRINGO_MSK - we now correctly
define this as inFo rather than inGo.
Now that we don't duplicate field information between _SHF & _MSK
definitions, and keep the fields next to the register accessors, it will
be much easier to spot & prevent any similar oddities being introduced
in the future.

Some CM registers are always 32 bits, or at least only use bits in the
lower 32 bits of the register. For these registers it is wasteful for us
to generate accessors which bother to check mips_cm_is64 & perform 64
bit accesses.
This patch modifies the accessor generation to take into account the
size of the register, and for 32 bit registers we generate accessors
which only ever perform 32 bit accesses. For 64 bit registers we either
perform a 64 bit access or two 32 bit accesses, depending upon the value
of mips_cm_is64. Doing this saves us ~1.5KiB of code in a generic 64r6el
kernel, and perhaps more importantly simplifies various code paths.
This removes the read64_gcr_* accessors, so mips_cm_error_report() is
modified to stop using them & instead use the regular read_gcr_*
accessors which will return 64 bit values from the 64 bit registers.
The new accessor macros are placed in asm/mips-cps.h such that they can
be shared by CPC & GIC code in later patches.

We currently have a mips_cm_base variable which holds the base address
of the Coherence Manager (CM) Global Configuration Registers (GCRs), and
accessor functions which use the GCR in their names. This works fine,
but gets in the way of sharing the code to generate the accessor
functions with other blocks (ie. CPC & GIC) because that code would then
need to separately handle the name of the base address variable & the
name used in the accessor functions.
In order to prepare for sharing the accessor generation code between CM,
CPC & GIC code this patch renames mips_cm_base to mips_gcr_base such
that the "gcr" portion is common to both the base address variable & the
accessor function names.

Rather than using BUG_ON in the case of an invalid attempt to lock
access to a non-zero VP on a pre-CM3 system, use WARN_ON so that we have
even the slightest chance of recovery.

CM3 provides a GCR_CL_OTHER register per VP, rather than only per core.
This means that we don't need to prevent other VPs within a core from
racing with code that makes use of the core-other register region.
Reduce locking overhead by demoting the per-core spinlock providing
protection for CM2.5 & lower to a per-CPU/per-VP spinlock for CM3 &
higher.

The use of config_enabled() against config options is ambiguous. In
practical terms, config_enabled() is equivalent to IS_BUILTIN(), but the
author might have used it for the meaning of IS_ENABLED(). Using
IS_ENABLED(), IS_BUILTIN(), IS_MODULE() etc. makes the intention
clearer.
This commit replaces config_enabled() with IS_ENABLED() where possible.
This commit is only touching bool config options.
I noticed two cases where config_enabled() is used against a tristate
option:
- config_enabled(CONFIG_HWMON)
[ drivers/net/wireless/ath/ath10k/thermal.c ]
- config_enabled(CONFIG_BACKLIGHT_CLASS_DEVICE)
[ drivers/gpu/drm/gma500/opregion.c ]
I did not touch them because they should be converted to IS_BUILTIN()
in order to keep the logic, but I was not sure it was the authors'
intention.

Ensure the update to which core the core-other GCR regions reflect has
taken place before any core-other GCRs are accessed by placing a memory
barrier (sync instruction) between the write to the core-other registers
and any such GCR accesses.

Introduce mips_cm_lock_other & mips_cm_unlock_other, mirroring the
existing CPC equivalents, in order to lock access from the current core
to another via the core-other GCR region. This hasn't been required in
the past but with CM3 the CPC starts using GCR_CL_OTHER rather than
CPC_CL_OTHER and this will be required for safety.

CM3 has 64 bit GCR_ERROR_* registers, but the code in
mips_cm_error_report was previously only reading 32 bits of it in MIPS32
kernels. Fix by splitting the reads for CM2 & CM3, and making use of the
read64_ variants of the accessor function for CM3.

Commit 3885c2b463f6 ("MIPS: CM: Add support for reporting CM cache
errors") added cases for decoding errors reported by CM3, but leaves the
buf variable which is printed as a string uninitialised for cause values
other than 1, 2 or 3. Fix by ensuring the buf variable is initialised to
an empty string in such cases.

Commit 3885c2b463f6 ("MIPS: CM: Add support for reporting CM cache
errors") leads to Malta boards unconditionally reading CM GCRs upon bus
errors, regardless of whether a CM is present. This is incorrect & will
lead to further exceptions. Fix by moving the GCR reads to after the
check for whether a CM is present.

The CM cache error reporting code is not Malta specific and as such it
should live in the mips-cm.c file. Moreover, CM2 and CM3 differ in the
way cache errors are being recorded to the registers so extend the
previous code to add support for the CM3 as well.

Previously, the CM accessors were only accessing CM registers as u32
types instead of using the native CM register with. However, newer CMs
may actually be 64-bit on MIPS64 cores. Fortunately, current 64-bit CMs
(CM3) hold all the useful configuration bits in the lower half of the
64-bit registers (at least most of them) so they can still be accessed
using the current 32-bit accessors.

Allow platforms to perform platform-specific steps before configuring
the L2 cache. This is necessary for platforms with CM3 since the L2
parameters no longer live in the Config2 register.

The kernel currently only probes for a MIPS Coherence Manager in the
Malta interrupt code in order to detect & enable the GIC. However CM is
not Malta-specific, so this should really be more generic. This patch
introduces some non-Malta-specific code which probes for a CM and
performs some basic initialisation.
A new header, with temporarily duplicated register definitions, is
introduced in order to:
1) Allow the new definitions to be correct with regards to the
CM documentation, as many of those in gcmpregs.h aren't.
2) Allow switching away from the REG() macro used via a few layers of
nested macros in order to access registers in gcmpregs.h. This
patch instead introduced accessor functions akin to the
{read,write}_c0_* functions used for cop0 registers.
3) Allow users of the CM to be migrated one by one.
4) Switch from the name 'GCMP' to 'CM' since the Coherence Manager is
what this code is actually dealing with.
