scsi/bnx2i: Convert to hotplug state machine

The real rework of this driver to use work queues is still stuck in review/testing on the SCSI mailing list. Signed-off-by: Sebastian Andrzej Siewior <bigeasy@linutronix.de> Cc: "James E.J.
