snd_msnd_pcm() takes a pointer to a pointer of a PCM where if this parameter
is provided the newly allocated PCM is stored. All callers pass NULL though,
so remove the parameter. This makes the code a bit cleaner and shorter.

Lots of sound drivers were getting module.h via the implicit presence
of it in <linux/device.h> but we are going to clean that up. So
fix up those users now.

Added SNDRV_PCM_INFO_BATCH flag to PCM info field of some drivers that
really don't give the precise pointer value.

This is driver for Turtle Beach Multisound cards:
Classic, Fiji and Pinnacle.
Tested pcm playback and recording and MIDI playback
on Multisound Pinnacle.
