[ Upstream commit 9b5948267adc9e689da609eb61cf7ed49cae5fa8 ] With external metadata device, flush requests are not passed down to the data device. Fix this by submitting the flush request in dm_integrity_flush_buffers. In order to not degrade performance, we overlap the data device flush with the metadata device flush. Move dm-bufio.h to include/linux/ so that external GPL'd DM target modules can use it. It is better to allow the use of dm-bufio than force external modules to implement the equivalent buffered IO mechanism in some new way. The hope is this will encourage the use of dm-bufio; which will then make it easier for a GPL'd external DM target module to be included upstream.
