The drivers/usb/gadget directory contains many files.
Files which are related can be distributed into separate directories.
This patch moves the USB functions implementations into a separate directory.

This adds multi-frame support to the NCM NTB's for
the gadget driver. This allows multiple network
packets to be put inside a single USB NTB with a
maximum size of 16kB.
It has a time out of 300ms to ensure that smaller
number of packets still maintain a normal latency.
Also the .fp_index and .next_fp_index have been
changed to .ndp_index and .next_ndp_index to
match the latest CDC-NCM specification and
help with maintenance.

Not a lot here during this merge window. Mostly we just have
the usual miscellaneous patches (removal of unnecessary prints,
proper dependencies being added to Kconfig, build warning fixes,
new device ID, etc.
Other than those, the only important new features are the
new support for OS Strings which should help Linux Gadget
Drivers behave better under MS Windows. Also Babble Recovery
implementation for MUSB on AM335x. Lastly, we also have
ARCH_QCOM PHY support though phy-msm.

In order to reduce the interrupt times in the embedded system,
a receiving workqueue is introduced.
This modification also enhanced the overall throughput as the
benefits of reducing interrupt occurrence.
This work was derived from previous work:
u_ether: move hardware transmit to RX workqueue.
Which should be base on codeaurora's work.
However, the benchmark on my platform shows the throughput
with workqueue is slightly better than NAPI.

Add an interface to create a struct netdev_dev filled with default values, an
interface which makes it an interface to fill the struct with useful values and
an interface to read the values set.
The patch also adds an interface to register the net device associated with an
ethernet-over-usb link.

u_ether.c has been #include'd by all gadgets which implement
USB Ethernet functions. In order to add configfs support,
the f_ecm.c, f_eem.c, f_ncm.c, f_subset.c, f_rndis.c need to be
converted into modules and must not be #include'd. Consequently,
the u_ether.c needs to be a module too, in a manner similar
to u_serial.c. The resulting module should not take any parameters,
so they are pushed to the current users of it, that is ether.c,
g_ffs.c, multi.c, ncm.c, nokia.c.

the u_ether.c file has a global variable named the_dev which keeps a
pointer to the network device after it has been created via
gether_setup_name(). It is only used internally by u_ether. This patches
moves the variable to its users and passes it via the port.ioport where
it is saved later anyway.

Checking skb->len against ETH_FRAME_LEN assumes a 1514
ethernet frame size. With an 802.1Q VLAN header, ethernet
frame length can now be 1518. Validate frame length against that.

Pull workqueue changes from Tejun Heo:
"This is workqueue updates for v3.7-rc1. A lot of activities this
round including considerable API and behavior cleanups.
* delayed_work combines a timer and a work item. The handling of the
timer part has always been a bit clunky leading to confusing
cancelation API with weird corner-case behaviors. delayed_work is
updated to use new IRQ safe timer and cancelation now works as
expected.
* Another deficiency of delayed_work was lack of the counterpart of
mod_timer() which led to cancel+queue combinations or open-coded
timer+work usages. mod_delayed_work[_on]() are added.
These two delayed_work changes make delayed_work provide interface
and behave like timer which is executed with process context.
* A work item could be executed concurrently on multiple CPUs, which
is rather unintuitive and made flush_work() behavior confusing and
half-broken under certain circumstances. This problem doesn't
exist for non-reentrant workqueues. While non-reentrancy check
isn't free, the overhead is incurred only when a work item bounces
across different CPUs and even in simulated pathological scenario
the overhead isn't too high.
All workqueues are made non-reentrant. This removes the
distinction between flush_[delayed_]work() and
flush_[delayed_]_work_sync(). The former is now as strong as the
latter and the specified work item is guaranteed to have finished
execution of any previous queueing on return.
* In addition to the various bug fixes, Lai redid and simplified CPU
hotplug handling significantly.
* Joonsoo introduced system_highpri_wq and used it during CPU
hotplug.
There are two merge commits - one to pull in IRQ safe timer from
tip/timers/core and the other to pull in CPU hotplug fixes from
wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."
Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

This pull request is large but the biggest part is the first part
of the cleanup on the gadget framework so we have a saner setup
to add configfs support for v3.8.
We have also some more conversions to the new udc_start/udc_stop
which makes us closer from dropping the old interfaces.
USB_GADGET_DUALSPEED and USB_GADGET_SUPERSPEED are finally gone,
thanks to Michal for his awesome work.
Other than that, we have the usual set of miscellaneous changes
and cleanups involving improvements to debug messages, removal
of duplicated includes, moving dereference after NULL test,
making renesas_hsbhs' irq handler Shared, unused code being dropped,
prevention of sleep-inside-spinlock bugs and a race condition fix
on udc-core.

This commit removes USB_GADGET_DUALSPEED and USB_GADGET_SUPERSPEED
Kconfig options. Since now kernel allows many UDC drivers to be
compiled, those options may turn to no longer be valid. For
instance, if someone decides to build UDC that supports super
speed and UDC that supports high speed only, the latter will be
"assumed" to support super speed since USB_GADGET_SUPERSPEED will
be selected by the former.
The test of whether CONFIG_USB_GADGET_*SPEED was defined was just
an optimisation which removed otherwise dead code (ie. if UDC is
not dual speed, there is no need to handle cases that can happen
if speed is high). This commit removes those checks.

flush[_delayed]_work_sync() are now spurious. Mark them deprecated
and convert all users to flush[_delayed]_work().
If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

This patch fixes an issue introduced by patch:
72c973d usb: gadget: add usb_endpoint_descriptor to struct usb_ep
Without this patch we see a kworker taking 100% CPU, after this sequence:
- Connect gadget to a windows host
- load g_ether
- ifconfig up <ip>; ifconfig down; ifconfig up
- ping <windows host>
The "ifconfig down" results in calling eth_stop(), which will call
usb_ep_disable() and, if the carrier is still ok, usb_ep_enable():

The ep should stay enabled, but will not, as ep_disable set the desc
pointer to NULL, therefore the subsequent ep_enable will fail. This leads
to permanent rescheduling of the eth_work() worker as usb_ep_queue()
(called by the worker) will fail due to the unconfigured endpoint.
We fix this issue by saving the ep descriptors and re-assign them before
usb_ep_enable().

A "usb0" interface that has never been connected to a host has an unknown
operstate, and therefore the IFF_RUNNING flag is (incorrectly) asserted
when queried by ifconfig, ifplugd, etc. This is a result of calling
netif_carrier_off() too early in the probe function; it should be called
after register_netdev().
Similar problems have been fixed in many other drivers, e.g.:

This optimization spares the FDs from saving the
endpoint chosen descriptor. This optimization is
not full though. To fully exploit this change, one
needs to update all the UDCs as well since in the
current implementation each of them saves the
endpoint descriptor in it's internal (and extended)
endpoint structure.

flush_scheduled_work() is being deprecated. Directly flush or cancel
work items instead.
* u_ether, isp1301_omap, speedtch conversions are straight-forward.
* ochi-hcd should only flush when quirk_nec() is true as otherwise the
work wouldn't have been initialized.
* In oti6858, cancel_delayed_work() + flush_scheduled_work() ->
cancel_delayed_work_sync().

NCM is a Network Control Model, subclass of USB CDC class,
specification is available at http://www.usb.org/developers/devclass_docs
This patch makes possible for u_ether to use multiply of wMaxPacketSize
predefined size transfers without ZLP (Zero Length Packet), required
by NCM spec.

Crash is triggered by commit e6484930d7 ("net: allocate tx queues in
register_netdevice"), which moved tx netqueue creation into register_netdev.
So now calling netif_stop_queue() before register_netdev causes an oops.
Move netif_stop_queue() after net device registration to fix crash.

__init, __initdata and __exit tags have have been removed from
various files to make it possible for gadgets that do not use
the __init/__exit tags to use those.
Files in question are related to:
* the core composite framework,
* the mass storage function (fixing a section mismatch) and
* ethernet driver (ACM, ECM, RNDIS).

implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

The problem with Ethernet based networking devices is to clearly
identify what's their usage. Special interfaces like bridges, WiFi,
Bluetooth, WiMAX or WWAN are already using DEVTYPE identification.
This patch marks the Ethernet functions of gadgets from the device
type "gadget". Automatic setup of these interfaces can now happen
from userspace without the need of hardcoding the network interface
name.

This patch adds a CDC EEM ethernet gadget driver. CDC EEM is a newer
USB ethernet specification that uses a simpler interface than the older
CDC ECM. This makes CDC EEM usable by a wider set of USB hardware.
By default the ethernet gadget will still use CDC ECM/Subset, but kernel
configuration and/or a module parameter will allow alternative use of
the CDC EEM protocol.
Changes since last version:
- Brought in missing RNDIS changes that caused compile error
- Modified 'sentinel CRC' checking to match EEM host driver

all ndo_start_xmit() return values of 0 to NETDEV_TX_OK.

Convert magic values 1 and -1 to NETDEV_TX_BUSY and NETDEV_TX_LOCKED respectively.
0 (NETDEV_TX_OK) is not changed to keep the noise down, except in very few cases
where its in direct proximity to one of the other values.

The g_ether USB gadget driver currently decides whether or not there's a
link to report back for eth_get_link based on if the USB link speed is
set. The USB gadget speed is however often set even before the device is
enumerated. It seems more sensible to only report a "link" if we're
actually connected to a host that wants to talk to us. The patch below
does this for me - tested with the PXA27x UDC driver.

This converts pretty much everything to print_mac. There were
a few things that had conflicts which I have just dropped for
now, no harm done.
I've built an allyesconfig with this and looked at the files
that weren't built very carefully, but it's a huge patch.

Bugfix for the new CDC Ethernet code: as part of activating the
network interface's USB link, make sure its link management code
knows whether the interface is open or not.
Without this fix, the link won't work right when it's brought up
before the link is active ... because the initial notification it
sends will have the wrong link state (down, not up). Makes it
hard to bridge these links (on the host side), among other things.
