2014-12-01Fix spurious request sense in error handlingJames Bottomley1-0/+9
commit d555a2abf3481f81303d835046a5ec2c4fb3ca8e upstream. We unconditionally execute scsi_eh_get_sense() to make sure all failed commands that should have sense attached, do. However, the routine forgets that some commands, because of the way they fail, will not have any sense code ... we should not bother them with a REQUEST_SENSE command. Fix this by testing to see if we actually got a CHECK_CONDITION return and skip asking for sense if we don't. This function is used by iscsi drivers and userspace to send iscsi PDUs/ commands. For login commands, we have a set buffer size. For all other commands we do not support data buffers. 2014-07-09sym53c8xx_2: Set DID_REQUEUE return code when aborting squeueMikulas Patocka1-0/+4
commit fd1232b214af43a973443aec6a2808f16ee5bf70 upstream. This patch fixes I/O errors with the sym53c8xx_2 driver when the disk returns QUEUE FULL status. When the controller encounters an error (including QUEUE FULL or BUSY status), it aborts all not yet submitted requests in the function sym_dequeue_from_squeue. This function aborts them with DID_SOFT_ERROR. If the disk has full tag queue, the request that caused the overflow is aborted with QUEUE FULL status (and the scsi midlayer properly retries it until it is accepted by the disk), but the sym53c8xx_2 driver aborts the following requests with DID_SOFT_ERROR --- for them, the midlayer does just a few retries and then signals the error up to sd. The result is that disk returning QUEUE FULL causes request failures. The error was reproduced on 53c895 with COMPAQ BD03685A24 disk (rebranded ST336607LC) with command queue 48 or 64 tags. The disk has 64 tags, but under some access patterns it return QUEUE FULL when there are less than 64 pending tags. The SCSI specification allows returning QUEUE FULL anytime and it is up to the host to retry. 2014-06-11isci: Fix a race condition in the SSP task management pathJeff Skirvin1-3/+6

The race is caused when an I/O termination in the SCU hardware is necessary because of an SSP target timeout condition, and the check of the I/O end state races against the HW-termination-driven end state. The failure of the race meant that no TMF was sent to the device to clean-up the pending I/O. On suspend, _scsih_suspend calls mpt2sas_base_free_resources, which in turn calls pci_disable_device if the device is enabled prior to suspending. However, _scsih_suspend also calls pci_disable_device itself. Thus, in the event that the device is enabled prior to suspending, pci_disable_device will be called twice. This patch removes the duplicate call to pci_disable_device in _scsi_suspend as it is both unnecessary and results in a kernel oops. In the first place, the loop 'for' in the macro 'for_each_isci_host' (drivers/scsi/isci/host.h:314) is incorrect, because it accesses the 3rd element of 2 element array. After the 2nd iteration it executes the instruction: ihost = to_pci_info(pdev)->hosts[2] (while the size of the 'hosts' array equals 2) and reads an out of range element. In the second place, this loop is incorrectly optimized by GCC v4.8 (see http://marc.info/?l=linux-kernel&m=138998871911336&w=2). 2014-03-24SCSI: isci: fix reset timeout handlingDan Williams2-8/+1
commit ddfadd7736b677de2d4ca2cd5b4b655368c85a7a upstream. Remove an erroneous BUG_ON() in the case of a hard reset timeout. The reset timeout handler puts the port into the "awaiting link-up" state. The timeout causes the device to be disconnected and we need to be in the awaiting link-up state to re-connect the port. The BUG_ON() made the incorrect assumption that resets never timeout and we always complete the reset in the "resetting" state. Testing this patch also uncovered that libata continues to attempt to reset the port long after the driver has torn down the context. Once the driver has committed to abandoning the link it must indicate to libata that recovery ends by returning -ENODEV from ->lldd_I_T_nexus_reset(). Bfa driver crash is observed while pushing the firmware on to chinook quad port card due to uninitialized bfi_image_ct2 access which gets initialized only for CT2 ASIC based cards after request_firmware(). For quard port chinook (CT2 ASIC based), bfi_image_ct2 is not getting initialized as there is no check for chinook PCI device ID before request_firmware and instead bfi_image_cb is initialized as it is the default case for card type check. This patch includes changes to read the right firmware for quad port chinook. 2013-12-12SCSI: hpsa: return 0 from driver probe function on success, not 1Stephen M. Cameron1-1/+1
commit 88bf6d62db4393fa03a58bada9d746312d5b496f upstream. A return value of 1 is interpreted as an error. See pci_driver. in local_pci_probe(). If you're wondering how this ever could have worked, it's because it used to be the case that only return values less than zero were interpreted as failure. But even in the current kernel if the driver registers its various entry points with the kernel, and then returns a value which is interpreted as failure, those registrations aren't undone, so the driver still mostly works. However, the driver's remove function wouldn't be called on rmmod, and pci power management functions wouldn't work. In the case of Smart Array, since it has a battery backed cache (or else no cache) even if the driver is not shut down properly as long as there is no outstanding i/o, nothing too bad happens, which is why it took so long to notice. Requesting backport to stable because the change to pci-driver.c which requires driver probe functions to return 0 occurred between 2.6.35 and 2.6.36 (the pci power management breakage) and again between 3.7 and 3.8 (pci_dev->driver getting set to NULL in local_pci_probe() preventing driver remove function from being called on rmmod.) Since commit 110dd8f19df5 "[SCSI] libsas: fix scr_read/write users and update the libata documentation" we have been passing pmp=1 and is_cmd=0 to ata_tf_to_fis(). Praveen reports that eSATA attached drives do not discover correctly. His investigation found that the BIOS was passing pmp=0 while Linux was passing pmp=1 and failing to discover the drives. Update libsas to follow the libata example of pulling the pmp setting from the ata_link and correct is_cmd to be 1 since all tf's submitted through ->qc_issue are commands. It appears that driver runs into a problem here if fibsize is too small because we allocate user_srbcmd with fibsize size only but later we access it until user_srbcmd->sg.count to copy it over to srbcmd. It is not correct to test (fibsize < sizeof(*user_srbcmd)) because this structure already includes one sg element and this is not needed for commands without data. So, we would recommend to add the following (instead of test for fibsize == 0). 2013-10-14esp_scsi: Fix tag state corruption when autosensing.David S. Miller2-6/+9
[ Upstream commit 21af8107f27878813d0364733c0b08813c2c192a ] Meelis Roos reports a crash in esp_free_lun_tag() in the presense of a disk which has died. The issue is that when we issue an autosense command, we do so by hijacking the original command that caused the check-condition. When we do so we clear out the ent->tag[] array when we issue it via find_and_prep_issuable_command(). This is so that the autosense command is forced to be issued non-tagged. That is problematic, because it is the value of ent->tag[] which determines whether we issued the original scsi command as tagged vs. non-tagged (see esp_alloc_lun_tag()). And that, in turn, is what trips up the sanity checks in esp_free_lun_tag(). That function needs the original ->tag[] values in order to free up the tag slot properly. Fix this by remembering the original command's tag values, and having esp_alloc_lun_tag() and esp_free_lun_tag() use them. When the loop ends, we know that "offset" lies beyond the end of the data in the buffer, so no Caching mode page was found. In theory it may be present, but the buffer size is limited to 512 bytes. Actually this issue needs fix in firmware, but for firmware running in field, this driver fix is proposed to resolve the issue. At firmware initialization time, if firmware does not come to ready state, driver will reset the adapter and retry for firmware transition to ready state unconditionally(not only executed for kdump kernel). 2013-08-04SCSI: sd: fix crash when UA received on DIF enabled deviceEwan D. Milne1-15/+7
commit 085b513f97d8d799d28491239be4b451bcd8c2c5 upstream. sd_prep_fn will allocate a larger CDB for the command via mempool_alloc for devices using DIF type 2 protection. This CDB was being freed in sd_done, which results in a kernel crash if the command is retried due to a UNIT ATTENTION. This change moves the code to free the larger CDB into sd_unprep_fn instead, which is invoked after the request is complete. It is no longer necessary to call scsi_print_command separately for this case as the ->cmnd will no longer be NULL in the normal code path. Also removed conditional test for DIF type 2 when freeing the larger CDB because the protection_type could have been changed via sysfs while the command was executing. Signed-off-by: Ewan D. commit 39c60a0948cc06139e2fbfe084f83cb7e7deae3b upstream. Some arrays synchronize their full non volatile cache when the sd driver sends a SYNCHRONIZE CACHE command. Unfortunately, they can have Terrabytes of this and we send a SYNCHRONIZE CACHE for every barrier if an array reports it has a writeback cache. This leads to massive slowdowns on journalled filesystems. The fix is to allow userspace to turn off the writeback cache setting as a temporary measure (i.e. without doing the MODE SELECT to write it back to the device), so even though the device reported it has a writeback cache, the user, knowing that the cache is non volatile and all they care about is filesystem correctness, can turn that bit off in the kernel and avoid the performance ruinous (and safety irrelevant) SYNCHRONIZE CACHE commands. 2013-02-14efi: Make 'efi_enabled' a function to query EFI facilitiesMatt Fleming1-1/+1
commit 83e68189745ad931c2afd45d8ee3303929233e7f upstream. Originally 'efi_enabled' indicated whether a kernel was booted from EFI firmware. Over time its semantics have changed, and it now indicates whether or not we are booted on an EFI machine with bit-native firmware, e.g. 64-bit kernel with 64-bit firmware. The immediate motivation for this patch is the bug report at, https://bugs.launchpad.net/ubuntu-cdimage/+bug/1040557 which details how running a platform driver on an EFI machine that is designed to run under BIOS can cause the machine to become bricked. Also, the following report, https://bugzilla.kernel.org/show_bug.cgi?id=47121 details how running said driver can also cause Machine Check Exceptions. Drivers need a new means of detecting whether they're running on an EFI machine, as sadly the expression, if (!efi_enabled) hasn't been a sufficient condition for quite some time. Users actually want to query 'efi_enabled' for different reasons - what they really want access to is the list of available EFI facilities. For instance, the x86 reboot code needs to know whether it can invoke the ResetSystem() function provided by the EFI runtime services, while the ACPI OSL code wants to know whether the EFI config tables were mapped successfully. There are also checks in some of the platform driver code to simply see if they're running on an EFI machine (which would make it a bad idea to do BIOS-y things). This patch is a prereq for the samsung-laptop fix patch. When the qla2xxx driver loses access to multiple, remote ports, there is a race condition which can occur which will keep the request stuck on a scsi request queue indefinitely. This bad state occurred do to a race condition with how the FCPORT_UPDATE_NEEDED bit is set in qla2x00_schedule_rport_del(), and how it is cleared in qla2x00_do_dpc(). The problem port has its drport pointer set, but it has never been processed by the driver to inform the fc transport that the port has been lost. qla2x00_schedule_rport_del() sets drport, and then sets the FCPORT_UPDATE_NEEDED bit. In qla2x00_do_dpc(), the port lists are walked and any drport pointer is handled and the fc transport informed of the port loss, then the FCPORT_UPDATE_NEEDED bit is cleared. This leaves a race where the dpc thread is processing one port removal, another port removal is marked with a call to qla2x00_schedule_rport_del(), and the dpc thread clears the bit for both removals, even though only the first removal was actually handled. Until another event occurs to set FCPORT_UPDATE_NEEDED, the later port removal is never finished and qla2xxx stays in a bad state which causes requests to become stuck on request queues. This patch updates the driver to test and clear FCPORT_UPDATE_NEEDED atomically. This ensures the port state changes are processed and not lost. The macro bit(n) is defined as ((u32)1 << n), and thus it doesn't work with n >= 32, such as in mvs_94xx_assign_reg_set(): if (i >= 32) { mvi->sata_reg_set |= bit(i); ... } The shift ((u32)1 << n) with n >= 32 also leads to undefined behavior. The result varies depending on the architecture. This patch changes bit(n) to do a 64-bit shift. It also simplifies mv_ffc64() using __ffs64(), since invoking ffz() with ~0 is undefined. commit 49bd665c5407a453736d3232ee58f2906b42e83c upstream. SATA MICROCODE DOWNALOAD fails on isci driver. After receiving Register Device to Host (FIS 0x34) frame Initiator resets phy. In the frame handler routine response (FIS 0x34) was copied into wrong buffer and upper layer did not receive any answer which resulted in timeout and reset. This patch corrects this bug. 2012-10-21SCSI: scsi_debug: Fix off-by-one bug when unmapping regionLukas Czerner1-1/+1
commit bc977749e967daa56de1922cf4cb38525631c51c upstream. Currently it is possible to unmap one more block than user requested to due to the off-by-one error in unmap_region(). This is probably due to the fact that the end variable despite its name actually points to the last block to unmap + 1. However in the condition it is handled as the last block of the region to unmap. The bug was not previously spotted probably due to the fact that the region was not zeroed, which has changed with commit be1dd78de5686c062bb3103f9e86d444a10ed783. With that commit we were able to corrupt the ext4 file system on 256M scsi_debug device with LBPRZ enabled using fstrim. Since the 'end' semantic is the same in several functions there this commit just fixes the condition to use the 'end' variable correctly in that context. In the absense of this patch, we could have a situation where the host may respond to a command that was issued prior to the issuance of the RESET command at some arbitrary time after responding to the RESET command.
