Xiaomi Poco F1 (qcom/sdm845-xiaomi-beryllium*.dts) comes with a SKhynix
H28U74301AMR UFS. The sd_read_cpr() operation leads to a 120 second
timeout, making the device bootup very slow:
[ 121.457736] sd 0:0:0:1: [sdb] tag#23 timing out command, waited 120s
Setting the BLIST_SKIP_VPD_PAGES allows the device to skip the failing
sd_read_cpr operation and boot normally.

Some storage, such as AIX VDASD (virtual storage) and IBM 2076 (front
end), fail as a result of commit c92a6b5d6335 ("scsi: core: Query VPD
size before getting full page").
That commit changed getting SCSI VPD pages so that we now read just
enough of the page to get the actual page size, then read the whole
page in a second read. The problem is that the above mentioned
hardware returns zero for the page size, because of a firmware
error. In such cases, until the firmware is fixed, this new blacklist
flag says to revert to the original method of reading the VPD pages,
i.e. try to read a whole buffer's worth on the first try.

Ultra HS-SD/MMC card reader devices establish a MEDIUM MAY HAVE CHANGED
unit attention not only when the medium changes but also when resuming from
suspend.
Setting the BLIST_IGN_MEDIA_CHANGE flag permits using runtime PM for these
readers.

On Fujitsu ETERNUS systems, sense code ABORTED COMMAND with ASC/Q C1/01
is used to indicate temporary condition where the storage-internal path
to a target is switched from one controller to another. SCSI commands
that return with this error code must be retried unconditionally
(i.e. without the "maybe_retry" logic in scsi_decide_disposition);
otherwise dm-multipath might initiate a failover from a healthy path
e.g. for REQ_FAILFAST_DEV commands.
Introduce a new blist flag for this case.

EMC Symmetrix returns 'internal target error' for a variety of
conditions, most of which will be transient. So we should always retry
it, even with failfast set. Otherwise we'd get spurious path flaps with
multipath.

Space for SCSI blist flags is gradually running out. Change the type to
__u64 and fix a checkpatch complaint about symbolic mode flags in
scsi_devinfo.c.
Make checkpatch happy by replacing simple_strtoul() with kstrtoull().

The Windows Server 2016 iSCSI target doesn't work with the Linux kernel
initiator since the kernel started sending larger requests by default,
nor does it implement the block limits VPD page. Apply the sector limit
workaround for these targets.

Commit 56f3d383f37b ("scsi: scsi_devinfo: Add TRY_VPD_PAGES to HITACHI
OPEN-V blacklist entry") modified some Hitachi entries:
HITACHI is always supporting VPD pages, even though it's claiming to
support SCSI Revision 3 only.
The same should have been done also for HP-rebranded.

Commit 627511e3e675 ("[SCSI] scsi_devinfo: update Hitachi entries (v2)")
modified some Hitachi entries:
Four models, OPEN-/DF400/DF500/DISK-SUBSYSTEM, can handle
REPORT_LUN, and the BLIST_REPORTLUN2 flag needs to be set. And DF600
doesn't require any flags because it returns ANSI 03h (SPC).
The same should have been done also for HP counterparts.

Since scsi_get_device_flags_keyed() callers do not check whether or not
the returned value is an error code, change that function such that it
returns a flags value even if the 'key' argument is invalid. Note:
since commit 28a0bc4120d3 ("scsi: sd: Implement blacklist option for
WRITE SAME w/ UNMAP") bit 31 is a valid device information flag so
checking whether bit 31 is set in the return value is not sufficient to
tell the difference between an error code and a flags value.

Cleanly fill memory for "vendor" and "model" with 0-bytes for the
"compatible" case rather than adding only a single 0 byte. This
simplifies the devinfo code a a bit, and avoids mistakes in other places
of the code (not in current upstream, but we had one such mistake in the
SUSE kernel).

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

The AIX VDASD devices do support VPD pages, but implement only SPC. So
set BLIST_TRY_VPD_PAGES to correctly display the VPD information in
sysfs.

When checking the model and vendor string we need to use the minimum
value of either string, otherwise we'll miss out on wildcard matches.
And we should take care when matching with zero size strings; results
might be unpredictable. With this patch the rules for matching devinfo
strings are as follows:
- Vendor strings must match exactly
- Empty Model strings will only match if the devinfo model
is also empty
- Model strings shorter than the devinfo model string will
not match

Commit b704f70ce200 ("SCSI: fix bug in scsi_dev_info_list matching")
changed the way vendor- and model-string matching was carried out in the
routine that looks up entries in a SCSI devinfo list. The new matching
code failed to take into account the case of a maximum-length string; in
such cases it could end up testing for a terminating '\0' byte beyond
the end of the memory allocated to the string. This out-of-bounds bug
was detected by UBSAN.
I don't know if anybody has actually encountered this bug. The symptom
would be that a device entry in the blacklist might not be matched
properly if it contained an 8-character vendor name or a 16-character
model name. Such entries certainly exist in scsi_static_device_list.
This patch fixes the problem by adding a check for a maximum-length
string before the '\0' test.

09e2b0b14690 ("scsi: rescan VPD attributes") added call to scsi_attach_vpd()
in scsi_rescan_device(). The commit causes the system to splat out

Without in-depth understanding of SCSI layer and the Marvell controller,
I suspect this happens because when the link goes down (because of an
error) we schedule scsi_rescan_device() which again fails to read VPD
data... ad infinitum.
Since VPD data cannot be read from the device anyway we prevent the SCSI
layer from even trying by blacklisting the device. This gets away the
error and the system starts up normally.

Another iscsi target that cannot handle large IOs, but does not tell us
a limit.
The Synology iSCSI targets report:

and the size of the command it can handle seems to depend on how much
memory it can allocate at the time. This results in IO errors when
handling large IOs. This patch just has us use the old 1024 default
sectors for this target by adding it to the scsi blacklist. We do not
have good contacs with this vendors, so I have not been able to try and
fix on their side.
I have posted this a long while back, but it was not merged. This
version just fixes it up for merge/patch failures in the original
version.

The "compatible" matching algorithm used for looking up old-style
blacklist entries in a scsi_dev_info_list is buggy. The core of the
algorithm looks like this:
if (memcmp(devinfo->vendor, vendor,
min(max, strlen(devinfo->vendor))))
/* not a match */
where max is the length of the device's vendor string after leading
spaces have been removed but trailing spaces have not. Because of the
min() computation, either entry could be a proper substring of the
other and the code would still think that they match.
In the case originally reported, the device's vendor and product
strings were "Inateck " and " ". These matched against
the following entry in the global device list:
{"", "Scanner", "1.80", BLIST_NOLUN}
because "" is a substring of "Inateck " and "" (the result of removing
leading spaces from the device's product string) is a substring of
"Scanner". The mistaken match prevented the system from scanning and
finding the device's second Logical Unit.
This patch fixes the problem by making two changes. First, the code
for leading-space removal is hoisted out of the loop. (This means it
will sometimes run unnecessarily, but since a large percentage of all
lookups involve the "compatible" entries in global device list, this
should be an overall improvement.) Second and more importantly, the
patch removes trailing spaces and adds a check to verify that the two
resulting strings are exactly the same length. This prevents matches
where one entry is a proper substring of the other.

In drivers/scsi/scsi_devinfo.c, the scsi_dev_info_list_del_keyed() and
scsi_get_device_flags_keyed() routines contain a large amount of
duplicate code for finding vendor/product matches in a
scsi_dev_info_list. This patch factors out the duplicate code and
puts it in a separate function, scsi_dev_info_list_find().

This works around a issue with qnap iscsi targets not handling large IOs
very well.
The target returns:

and it is *sometimes* able to handle at least one IO of size up to 8 MB. We
have seen in traces where it will sometimes work, but other times it
looks like it fails and it looks like it returns failures if we send
multiple large IOs sometimes. Also it looks like it can return 2 different
errors. It will sometimes send iscsi reject errors indicating out of
resources or it will send invalid cdb illegal requests check conditions.
And then when it sends iscsi rejects it does not seem to handle retries
when there are command sequence holes, so I could not just add code to
try and gracefully handle that error code.
The problem is that we do not have a good contact for the company,
so we are not able to determine under what conditions it returns
which error and why it sometimes works.
So, this patch just adds a new black list flag to set targets like this to
the old max safe sectors of 1024. The max_hw_sectors changes added in 3.19
caused this regression, so I also ccing stable.
