A static code analysis tool flagged the possibility of buffer overflow when
using copy_from_user() for a debugfs entry.
Currently, it is possible that copy_from_user() copies more bytes than what
would fit in the mybuf char array. Add a min() restriction check between
sizeof(mybuf) - 1 and nbytes passed from the userspace buffer to protect
against buffer overflow.

When if_type equals zero and pci_resource_start(pdev, PCI_64BIT_BAR4)
returns false, drbl_regs_memmap_p is not remapped. This passes a NULL
pointer to iounmap(), which can trigger a WARN() on certain arches.
When if_type equals six and pci_resource_start(pdev, PCI_64BIT_BAR4)
returns true, drbl_regs_memmap_p may has been remapped and
ctrl_regs_memmap_p is not remapped. This is a resource leak and passes a
NULL pointer to iounmap().
To fix these issues, we need to add null checks before iounmap(), and
change some goto labels.

If the &epd_pool->list is empty when executing
lpfc_get_io_buf_from_expedite_pool() the function would return an invalid
pointer. Even in the case if the list is guaranteed to be populated, the
iterator variable should not be used after the loop to be more robust for
future changes.
Linus proposed to avoid any use of the list iterator variable after the
loop, in the attempt to move the list iterator variable declaration into
the macro to avoid any potential misuse after the loop [1].

After issuing a LIP, a specific target vendor does not ACC the FLOGI that
lpfc sends. However, it does send its own FLOGI that lpfc ACCs. The
target then establishes the port IDs by sending a PLOGI. lpfc PLOGI_ACCs
and starts the RPI registration for DID 0x000001. The target then sends a
LOGO to the fabric DID. lpfc is currently treating the LOGO from the
fabric DID as a link down and cleans up all the ndlps. The ndlp for DID
0x000001 is put back into NPR and discovery stops, leaving the port in
stuck in bypassed mode.
Change lpfc behavior such that if a LOGO is received for the fabric DID in
PT2PT topology skip the lpfc_linkdown_port() routine and just move the
fabric DID back to NPR.

If no handler is found in lpfc_complete_unsol_iocb() to match the rctl of a
received frame, the frame is dropped and resources are leaked.
Fix by returning resources when discarding an unhandled frame type. Update
lpfc_fc_frame_check() handling of NOP basic link service.

When recovering from a pci-parity error the driver is failing to re-create
queues, causing recovery to fail. Looking deeper, it was found that the
interrupt vector count allocated on the recovery was fewer than the vectors
originally allocated. This disparity resulted in CPU map entries with stale
information. When the driver tries to re-create the queues, it attempts to
use the stale information which indicates an eq/interrupt vector that was
no longer created.
Fix by clearng the cpup map array before enabling and requesting the IRQs
in the lpfc_sli_reset_slot_s4 routine().

When connected point to point, the driver does not know the FC4's supported
by the other end. In Fabrics, it can query the nameserver. Thus the driver
must send PRLIs for the FC4s it supports and enable support based on the
acc(ept) or rej(ect) of the respective FC4 PRLI. Currently the driver
supports SCSI and NVMe PRLIs.
Unfortunately, although the behavior is per standard, many devices have
come to expect only SCSI PRLIs. In this particular example, the NVMe PRLI
is properly RJT'd but the target decided that it must LOGO after seeing the
unexpected NVMe PRLI. The LOGO causes the sequence to restart and login is
now in an infinite failure loop.
Fix the problem by having the driver, on a pt2pt link, remember NVMe PRLI
accept or reject status across logout as long as the link stays "up". When
retrying login, if the prior NVMe PRLI was rejected, it will not be sent on
the next login.

Contention for the mailbox interface may occur during driver initialization
(immediately after a function reset), between mailbox commands initiated
via ioctl (bsg) and those driver requested by the driver.
After setting SLI_ACTIVE flag for a port, there is a window in which the
driver will allow an ioctl to be initiated while the adapter is
initializing and issuing mailbox commands via polling. The polling logic
then gets confused.
Correct by having thread setting SLI_ACTIVE spot an active mailbox command
and allow it complete before proceeding.

Messages around firmware download were incorrectly tagged as being related
to discovery trace events. Thus, firmware download status ended up dumping
the trace log as well as the firmware update message. As there were a
couple of log messages in this state, the trace log was dumped multiple
times.
Resolve this by converting from trace events to SLI events.

The driver is initiating NVMe PRLIs to determine device NVMe support. This
should not be occurring if CONFIG_NVME_FC support is disabled.
Correct this by changing the default value for FC4 support. Currently it
defaults to FCP and NVMe. With change, when NVME_FC support is not enabled
in the kernel, the default value is just FCP.

Extraneous teardown routines are present in the firmware dump path causing
altered states in firmware captures.
When a firmware dump is requested via sysfs, trigger the dump immediately
without tearing down structures and changing adapter state.
The driver shall rely on pre-existing firmware error state clean up
handlers to restore the adapter.

When parsing the txq list in lpfc_drain_txq(), the driver attempts to pass
the requests to the adapter. If such an attempt fails, a local "fail_msg"
string is set and a log message output. The job is then added to a
completions list for cancellation.
Processing of any further jobs from the txq list continues, but since
"fail_msg" remains set, jobs are added to the completions list regardless
of whether a wqe was passed to the adapter. If successfully added to
txcmplq, jobs are added to both lists resulting in list corruption.
Fix by clearing the fail_msg string after adding a job to the completions
list. This stops the subsequent jobs from being added to the completions
list unless they had an appropriate failure.

Code review found this to be an errant failure path that was taken, jumping

An 'unexpected timeout' message may be seen in a point-2-point topology.
The message occurs when a PLOGI is received before the driver is notified
of FLOGI completion. The FLOGI completion failure causes discovery to be
triggered for a second time. The discovery timer is restarted but no new
discovery activity is initiated, thus the timeout message eventually
appears.
In point-2-point, when discovery has progressed before the FLOGI completion
is processed, it is not a failure. Add code to FLOGI completion to detect
that discovery has progressed and exit the FLOGI handling (noop'ing it).

In devloss timer handler and in backend calls to terminate remote port I/O,
there is logic to walk through all active IOCBs and validate them to
potentially trigger an abort request. This logic is causing illegal memory
accesses which leads to a crash. Abort IOCBs, which may be on the list, do
not have an associated lpfc_io_buf struct. The driver is trying to map an
lpfc_io_buf struct on the IOCB and which results in a bogus address thus
the issue.
Fix by skipping over ABORT IOCBs (CLOSE IOCBs are ABORTS that don't send
ABTS) in the IOCB scan logic.

SLI-4 does not contain a PORT_CAPABILITIES mailbox command (only SLI-3
does, and SLI-3 doesn't use it), yet there are SLI-4 code paths that have
code to issue the command. The command will always fail.
Remove the code for the mailbox command and leave only the resulting
"failure path" logic.

In SLI-4, when performing a mailbox command with MBX_POLL, the driver uses
the BMBX register to send the command rather than the MQ. A flag is set
indicating the BMBX register is active and saves the mailbox job struct
(mboxq) in the mbox_active element of the adapter. The routine then waits
for completion or timeout. The mailbox job struct is not freed by the
routine. In cases of timeout, the adapter will be reset. The
lpfc_sli_mbox_sys_flush() routine will clean up the mbox in preparation for
the reset. It clears the BMBX active flag and marks the job structure as
MBX_NOT_FINISHED. But, it never frees the mboxq job structure. Expectation
in both normal completion and timeout cases is that the issuer of the mbx
command will free the structure. Unfortunately, not all calling paths are
freeing the memory in cases of error.
All calling paths were looked at and updated, if missing, to free the mboxq
memory regardless of completion status.

Fix a crash caused by a double put on the node when the driver completed an
ACC for an unsolicted abort on the same node. The second put was executed
by lpfc_nlp_not_used() and is wrong because the completion routine executes
the nlp_put when the iocbq was released. Additionally, the driver is
issuing a LOGO then immediately calls lpfc_nlp_set_state to put the node
into NPR. This call does nothing.
Remove the lpfc_nlp_not_used call and additional set_state in the
completion routine. Remove the lpfc_nlp_set_state post issue_logo. Isn't
necessary.

On a pt2pt setup, between 2 initiators, if one side issues a a LOGO, there
is no relogin attempt. The FC specs are grey in this area on which port
(higher wwn or not) is to re-login.
As there is no spec guidance, unconditionally re-PLOGI after the logout to
ensure a login is re-established.

The wqe_dbde field indicates whether a Data BDE is present in Words 0:2 and
should therefore should be clear in the abts request wqe. By setting the
bit we can be misleading fw into error cases.

Commit 9816ef6ecbc1 ("scsi: lpfc: Use after free in lpfc_rq_buf_free()")
was made to correct a use after free condition in lpfc_rq_buf_free().
Unfortunately, a subsequent patch cut on a tree without the fix
inadvertently reverted the fix.
Put the fix back: Move the freeing of the rqb_entry to after the print
function that references it.

With the conversion to blk_io_poll for better interrupt latency in normal
cases, it introduced this code path, executed when I/O aborts or logouts
are seen, which attempts to allocate memory for a mailbox command to be
issued. The allocation is GFP_KERNEL, thus it could attempt to sleep.
Fix by creating a work element that performs the event handling for the
remote port. This will have the mailbox commands and other items performed
in the work element, not the irq. A much better method as the "irq" routine
does not stall while performing all this deep handling code.
Ensure that allocation failures are handled and send LOGO on failure.
Additionally, enlarge the mailbox memory pool to reduce the possibility of
additional allocation in this path.

A prior patch introduced a spin_lock_irqsave(hbalock) in the
lpfc_post_rq_buffer() routine. Call trace is seen as the hbalock is held

Initial FLOGIs are failing with the following message:
lpfc 0000:13:00.1: 1:(0):0820 FLOGI Failed (x300). BBCredit Not Supported
In a prior patch, the READ_SPARAM command was re-ordered to post after
CONFIG_LINK as the driver is expected to update the driver's copy of the
service parameters for the FLOGI payload. If the bb-credit recovery feature
is enabled, this is fine. But on adapters were bb-credit recovery isn't
enabled, it would cause the FLOGI to fail.
Fix by restoring the original command order (READ_SPARAM before
CONFIG_LINK), and after issuing CONFIG_LINK, detect bb-credit recovery
support and reissuing READ_SPARAM to obtain the updated service parameters
(effectively adding in the fix command order).

The driver is unable to successfully login with remote device. During pt2pt
login, the driver completes its FLOGI request with the remote device having
WWN precedence. The remote device issues its own (delayed) FLOGI after
accepting the driver's and, upon transmitting the FLOGI, immediately
recognizes it has already processed the driver's FLOGI thus it transitions
to sending a PLOGI before waiting for an ACC to its FLOGI.
In the driver, the FLOGI is received and an ACC sent, followed by the PLOGI
being received and an ACC sent. The issue is that the PLOGI reception
occurs before the response from the adapter from the FLOGI ACC is
received. Processing of the PLOGI sets state flags to perform the REG_RPI
mailbox command and proceed with the rest of discovery on the port. The
same completion routine used by both FLOGI and PLOGI is generic in
nature. One of the things it does is clear flags, and those flags happen to
drive the rest of discovery. So what happened was the PLOGI processing set
the flags, the FLOGI ACC completion cleared them, thus when the PLOGI ACC
completes it doesn't see the flags and stops.
Fix by modifying the generic completion routine to not clear the rest of
discovery flag (NLP_ACC_REGLOGIN) unless the completion is also associated
with performing a mailbox command as part of its handling. For things such
as FLOGI ACC, there isn't a subsequent action to perform with the adapter,
thus there is no mailbox cmd ptr. PLOGI ACC though will perform REG_RPI
upon completion, thus there is a mailbox cmd ptr.

Some systems are reporting the following log message during driver unload
or system shutdown:
ics_rtas_set_affinity: No online cpus in the mask
A prior commit introduced the writing of an empty affinity mask in calls to
irq_set_affinity_hint() when disabling interrupts or when there are no
remaining online CPUs to service an eq interrupt. At least some ppc64
systems are checking whether affinity masks are empty or not.
Do not call irq_set_affinity_hint() with an empty CPU mask.

Pull SCSI updates from James Bottomley:
"This consists of the usual driver updates (ufs, qla2xxx, tcmu, lpfc,
hpsa, zfcp, scsi_debug) and minor bug fixes.
We also have a huge docbook fix update like most other subsystems and
no major update to the core (the few non trivial updates are either
minor fixes or removing an unused feature [scsi_sdb_cache])"
