Fix a target login issue, when parent interface is vlan and we are
using cxgb3i sepecific private ip address in '/etc/iscsi/ifaces/'

This implements warm target reset tmf support for
the scsi-ml target reset callback. Previously we would
just drop the session in that callback. This patch will
now try a target reset and if that fails drop the session.

Massage the err_handler upcall into an event handler upcall, pass

There is a bug when VLAN is configured on the cxgb3 interface, the iscsi
conn. would be denied with message "cxgb3i: NOT going through cxgbi device."
This patch adds code to get the real egress net_device when vlan is configured.

The default kernel pages supported are 4K, 8K, 16K, and 64K. Re-calculate
entries if PAGE_SIZE is not one of the defaults.

add a refcnt to keep track of it, so we would not release it pre-maturely.

This driver needs dst_cache->dev so it should include net/dst.h
to ensure that it builds. While net/tcp.h probably includes it
already, we shouldn't rely on that since there is no guarantee
that this won't change in future.

When we create the tcp/ip connection by calling ep_connect, we currently
just go by the routing table info.
I think there are two problems with this.
1. Some drivers do not have access to a routing table. Some drivers like
qla4xxx do not even know about other ports.
2. If you have two initiator ports on the same subnet, the user may have
set things up so that session1 was supposed to be run through port1. and
session2 was supposed to be run through port2. It looks like we could
end with both sessions going through one of the ports.

When doing a lot (128) of large writes (256K) we can hit the cxgb3_snd_win
check pretty easily. The driver's xmit thread then takes 100% of the cpu.
The driver should not be returning -EAGAIN for this problem. It should
be returing -ENOBUFS, then when the window is opened again it should
queue the xmit thread (it already wakes the xmit thread).

cxgb3i was setting can_queue to only 128 commands, and was
setting the can_queue and cmd_per_lun to the same value.
This sets the can_queue to 1024 commands, and sets the cmd_per_lun
to a safer default of 32.

Set target can queue limit to the number of preallocated
session tasks we have.
This along with the cxgb3i can_queue patch will fix a throughput
problem where it could only queue one LU worth of data at a time.

cxgb3i_ddp_cleanup just calls ddp_release directly so there is
no reason for the wrapper. This patch just renames ddp_release
to cxgb3i_ddp_cleanup and removes the old wrapper function.

We do not need to have llds set the host no for the session's
parent, because we know the session's parent is going to be
the host. This removes it from the session creation callback
and converts the drivers.

The qdepth setting was useful when we needed libiscsi to verify
the setting. Now we just need to make sure if older tools
passed in zero then we need to set some default.
So this patch just has us use the sht->cmd_per_lun or if
for LLD does a host per session then we can set it on per
host basis.

We were using the shost work queue which ended up being
a little akward since all iscsi hosts need a thread for
scanning, but only drivers hooked into libiscsi need
a workqueue for transmitting. So this patch moves the
xmit workqueue to the lib.

Need to make sure the outgoing pdu can fit into a single skb. When
calulating the max. outgoing pdu payload size, take into consideration
of
- data can be held in the skb's fragment list, assume 512 bytes per
fragment, and
- data can be held in the headroom.

- resize the work-request credit array to be based on skb's MAX_SKB_FRAGS.
- split the skb cb into tx and rx portion
- increase the default transmit window to 128K.
- stop queueing up the outgoing pdus if transmit window is full.

cxgb3i requires the cxgb3 net driver, so it selects it. However,
cxgb3 has dependencies which the select cannot see. Fix this by
separating out the cxgb3 dependencies into a separate hidden config
option (CONFIG_CHELSIO_T3_DEPENDS) and make both cxgb3 and cxgb3i
depend on it.

The cxgb3i was using skb->sp pointer for some internal book-keeping
which is not related to the secure path. Changed it to use skb->cb[]
instead.

This patch implements the cxgb3i iscsi connection acceleration for the
open-iscsi initiator.
The cxgb3i driver offers the iscsi PDU based offload:
- digest insertion and verification
- payload direct-placement into host memory buffer.
