After a copy pair swap the block device's "device" symlink points to
the secondary CCW device, but the gendisk's parent remained the
primary, leaving /sys/block/<dasdx> under the wrong parent.
Move the gendisk to the secondary's device with device_move(), keeping
the sysfs topology consistent after the swap.

The function 'mpc_rcvd_sweep_req(mpcginfo)' is called conditionally
from function 'ctcmpc_unpack_skb'. It frees passed mpcginfo.
After that a call to function 'kfree' in function 'ctcmpc_unpack_skb'
frees it again.
Remove 'kfree' call in function 'mpc_rcvd_sweep_req(mpcginfo)'.
Bug detected by the clang static analyzer.

Starting with 'commit 2297791c92d0 ("s390/cio: dont unregister
subchannel from child-drivers")', cio no longer unregisters
subchannels when the attached device is invalid or unavailable.
As an unintended side-effect, the cio_ignore purge function no longer
removes subchannels for devices on the cio_ignore list if no CCW device
is attached. This situation occurs when a CCW device is non-operational
or unavailable
To ensure the same outcome of the purge function as when the
current cio_ignore list had been active during boot, update the purge
function to remove I/O subchannels without working CCW devices if the
associated device number is found on the cio_ignore list.

Tracing code called by the SCLP interrupt handler contains early exits
if the SCCB address associated with an interrupt is NULL. This check is
performed after physical to virtual address translation.
If the kernel identity mapping does not start at address zero, the
resulting virtual address is never zero, so that the NULL checks won't
work. Subsequently this may result in incorrect accesses to the first
page of the identity mapping.
Fix this by introducing a function that handles the NULL case before
address translation.

The s390x ISM device data sheet clearly states that only one
request-response sequence is allowable per ISM function at any point in
time. Unfortunately as of today the s390/ism driver in Linux does not
honor that requirement. This patch aims to rectify that.
This problem was discovered based on Aliaksei's bug report which states
that for certain workloads the ISM functions end up entering error state
(with PEC 2 as seen from the logs) after a while and as a consequence
connections handled by the respective function break, and for future
connection requests the ISM device is not considered -- given it is in a
dysfunctional state. During further debugging PEC 3A was observed as
well.
A kernel message like
[ 1211.244319] zpci: 061a:00:00.0: Event 0x2 reports an error for PCI function 0x61a
is a reliable indicator of the stated function entering error state
with PEC 2. Let me also point out that a kernel message like
[ 1211.244325] zpci: 061a:00:00.0: The ism driver bound to the device does not support error recovery
is a reliable indicator that the ISM function won't be auto-recovered
because the ISM driver currently lacks support for it.
On a technical level, without this synchronization, commands (inputs to
the FW) may be partially or fully overwritten (corrupted) by another CPU
trying to issue commands on the same function. There is hard evidence that
this can lead to DMB token values being used as DMB IOVAs, leading to
PEC 2 PCI events indicating invalid DMA. But this is only one of the
failure modes imaginable. In theory even completely losing one command
and executing another one twice and then trying to interpret the outputs
as if the command we intended to execute was actually executed and not
the other one is also possible. Frankly, I don't feel confident about
providing an exhaustive list of possible consequences.

Number of apqn target list entries contained in 'nr_apqns' variable is
determined by userspace via an ioctl call so the result of the product in
calculation of size passed to memdup_user() may overflow.
In this case the actual size of the allocated area and the value
describing it won't be in sync leading to various types of unpredictable
behaviour later.
Use a proper memdup_array_user() helper which returns an error if an
overflow is detected. Note that it is different from when nr_apqns is
initially zero - that case is considered valid and should be handled in
subsequent pkey_handler implementations.
Found by Linux Verification Center (linuxtesting.org).

Improve the usability of the unit_add sysfs attribute by ensuring that
the associated FCP LUN scan processing is completed synchronously. This
enables configuration tooling to consistently determine the end of the
scan process to allow for serialization of follow-on actions.
While the scan process associated with unit_add typically completes
synchronously, it is deferred to an asynchronous background process if
unit_add is used before initial remote port scanning has completed. This
occurs when unit_add is used immediately after setting the associated FCP
device online.
To ensure synchronous unit_add processing, wait for remote port scanning
to complete before initiating the FCP LUN scan.

An erroneous message is written to the kernel log when either of the
following actions are taken by a user:
1. Assign an adapter or domain to a vfio_ap mediated device via its sysfs
assign_adapter or assign_domain attributes that would result in one or
more AP queues being assigned that are already assigned to a different
mediated device. Sharing of queues between mdevs is not allowed.
2. Reserve an adapter or domain for the host device driver via the AP bus
driver's sysfs apmask or aqmask attribute that would result in providing
host access to an AP queue that is in use by a vfio_ap mediated device.
Reserving a queue for a host driver that is in use by an mdev is not
allowed.
In both cases, the assignment will return an error; however, a message like
the following is written to the kernel log:
vfio_ap_mdev e1839397-51a0-4e3c-91e0-c3b9c3d3047d: Userspace may not
re-assign queue 00.0028 already assigned to \

Notice the mdev reporting the error is the same as the mdev identified
in the message as the one to which the queue is being assigned.
It is perfectly okay to assign a queue to an mdev to which it is
already assigned; the assignment is simply ignored by the vfio_ap device
driver.
This patch logs more descriptive and accurate messages for both 1 and 2
above to the kernel log:

In some environments, the SCLP firmware interface used to query a
CHPID's configured state is not supported. On these environments,
rapidly reading the corresponding sysfs attribute produces inconsistent
results:
$ cat /sys/devices/css0/chp0.00/configure
cat: /sys/devices/css0/chp0.00/configure: Operation not supported
$ cat /sys/devices/css0/chp0.00/configure

This occurs for example when Linux is run as a KVM guest. The
inconsistency is a result of CIO using cached results for generating
the value of the "configure" attribute while failing to handle the
situation where no data was returned by SCLP.
Fix this by not updating the cache-expiration timestamp when SCLP
returns no data. With the fix applied, the system response is
consistent:

According to device_release() in /drivers/base/core.c,
a device without a release function is a broken device
and must be fixed.
The current code directly frees the device after calling device_add()
without waiting for other kernel parts to release their references.
Thus, a reference could still be held to a struct device,
e.g., by sysfs, leading to potential use-after-free
issues if a proper release function is not set.

According to virtual ISM support feature defined by SMCv2.1, GIDs of
virtual ISM device are UUIDs defined by RFC4122, which are 128-bits
long. So some adaptation work is required. And note that the GIDs of
existing platform firmware ISM devices still remain 64-bits long.

Starting with commit 2297791c92d0 ("s390/cio: dont unregister
subchannel from child-drivers"), CIO does not unregister subchannels
when the attached device is invalid or unavailable. Instead, it
allows subchannels to exist without a connected device. However, if
the DNV value is 0, such as, when all the CHPIDs of a subchannel are
configured in standby state, the subchannel is unregistered, which
contradicts the current subchannel specification.
Update the logic so that subchannels are not unregistered based
on the DNV value. Also update the SCHIB information even if the
DNV bit is zero.

According to the VT220 specification the possible character combinations
sent on RETURN are only CR or CRLF [0].
The Return key sends either a CR character (0/13) or a CR
character (0/13) and an LF character (0/10), depending on the
set/reset state of line feed/new line mode (LNM).
The sclp/vt220 driver however uses LFCR. This can confuse tools, for
example the kunit runner.

On reboot the SCLP interface is deactivated through a reboot notifier.
This happens before other components using SCLP have the chance to run
their own reboot notifiers.
Two of those components are the SCLP console and tty drivers which try
to flush the last outstanding messages.
At that point the SCLP interface is already unusable and the messages
are discarded.
Execute sclp_deactivate() as late as possible to avoid this issue.

This reverts commit bc792884b76f ("s390/dasd: Establish DMA alignment").
Quoting the original commit:
linux-next commit bf8d08532bc1 ("iomap: add support for dma aligned
direct-io") changes the alignment requirement to come from the block
device rather than the block size, and the default alignment
requirement is 512-byte boundaries. Since DASD I/O has page
alignments for IDAW/TIDAW requests, let's override this value to
restore the expected behavior.
I mentioned TIDAW, but that was wrong. TIDAWs have no distinct alignment
requirement (per p. 15-70 of POPS SA22-7832-13):
Unless otherwise specified, TIDAWs may designate
a block of main storage on any boundary and length
up to 4K bytes, provided the specified block does not
cross a 4 K-byte boundary.
IDAWs do, but the original commit neglected that while ECKD DASD are
typically formatted in 4096-byte blocks, they don't HAVE to be. Formatting
an ECKD volume with smaller blocks is permitted (dasdfmt -b xxx), and the
problematic commit enforces alignment properties to such a device that
will result in errors, such as:

The original commit omitted the FBA discipline for just this reason,
but the formatted block size of the other disciplines was overlooked.
The solution to all of this is to revert to the original behavior,
such that the block size can be respected.
But what of the original problem? That was manifested with a direct-io
QEMU guest, where QEMU itself was changed a month or two later with
commit 25474d90aa ("block: use the request length for iov alignment")
such that the blamed kernel commit is unnecessary.

The number of times yet another open coded
`BITS_TO_LONGS(nbits) * sizeof(long)` can be spotted is huge.
Some generic helper is long overdue.
Add one, bitmap_size(), but with one detail.
BITS_TO_LONGS() uses DIV_ROUND_UP(). The latter works well when both
divident and divisor are compile-time constants or when the divisor
is not a pow-of-2. When it is however, the compilers sometimes tend
to generate suboptimal code (GCC 13):

Clang does it better and generates the same code before/after starting
from -O1, except that with the ALIGN() approach it uses %edx and thus
still saves some bytes:
add/remove: 0/0 grow/shrink: 9/133 up/down: 18/-538 (-520)
Note that we can't expand DIV_ROUND_UP() by adding a check and using
this approach there, as it's used in array declarations where
expressions are not allowed.
Add this helper to tools/ as well.

bitmap_size() is a pretty generic name and one may want to use it for
a generic bitmap API function. At the same time, its logic is not
"generic", i.e. it's not just `nbits -> size of bitmap in bytes`
converter as it would be expected from its name.
Add the prefix 'idset_' used throughout the file where the function
resides.

Extent Space Efficient (ESE) or thin provisioned volumes need to be
formatted on demand during usual IO processing.
The dasd_ese_needs_format function checks for error codes that signal
the non existence of a proper track format.
The check for incorrect length is to imprecise since other error cases
leading to transport of insufficient data also have this flag set.
This might lead to data corruption in certain error cases for example
during a storage server warmstart.
Fix by removing the check for incorrect length and replacing by
explicitly checking for invalid track format in transport mode.
Also remove the check for file protected since this is not a valid
ESE handling case.

When a task waiting for completion of a Store Data operation is
interrupted, an attempt is made to halt this operation. If this attempt
fails due to a hardware or firmware problem, there is a chance that the
SCLP facility might store data into buffers referenced by the original
operation at a later time.
Handle this situation by not releasing the referenced data buffers if
the halt attempt fails. For current use cases, this might result in a
leak of few pages of memory in case of a rare hardware/firmware
malfunction.

dasd_add_busid() can return an error via ERR_PTR() if an allocation
fails. However, two callsites in dasd_copy_pair_store() do not check
the result, potentially resulting in a NULL pointer dereference. Fix
this by checking the result with IS_ERR() and returning the error up
the stack.

occured when /sys/bus/ap/a[pq]mask was updated with a relative mask value
(like +0x10-0x12,+60,-90) with one of the numeric values exceeding INT_MAX.
The fix is simple: use unsigned long values for the internal variables. The
correct checks are already in place in the function but a simple int for
the internal variables was used with the possibility to overflow.

Analysis:
There is one napi structure per out_q: card->qdio.out_qs[i].napi
The napi.poll functions are set during qeth_open().
Since
commit 1cfef80d4c2b ("s390/qeth: Don't call dev_close/dev_open (DOWN/UP)")
qeth_set_offline()/qeth_set_online() no longer call dev_close()/
dev_open(). So if qeth_free_qdio_queues() cleared
card->qdio.out_qs[i].napi.poll while the network interface was UP and the
card was offline, they are not set again.
Reproduction:

Note that a Completion Queue (CQ) is only enabled or disabled, when hsuid
is set for the first time or when it is removed.
Workarounds:
- Set hsuid before setting the device online for the first time
or
- Use chzdev -d $devno; chzdev $devno hsuid=xxx; chzdev -e $devno;
to set hsuid on an existing device. (this will remove and recreate the
network interface)
Fix:
There is no need to free the output queues when a completion queue is
added or removed.
card->qdio.state now indicates whether the inbound buffer pool and the
outbound queues are allocated.
card->qdio.c_q indicates whether a CQ is allocated.

Currently, we allocate a lbuf-sized kernel buffer and copy lbuf from
userspace to that buffer. Later, we use scanf on this buffer but we don't
ensure that the string is terminated inside the buffer, this can lead to
OOB read when using scanf. Fix this issue by using memdup_user_nul instead.

A race condition exists in ccw_device_set_online() that can cause the
online process to fail, leaving the affected device in an inconsistent
state. As a result, subsequent attempts to set that device online fail
with return code ENODEV.
The problem occurs when a path verification request arrives after
a wait for final device state completed, but before the result state
is evaluated.
Fix this by ensuring that the CCW-device lock is held between
determining final state and checking result state.
Note that since:
commit 2297791c92d0 ("s390/cio: dont unregister subchannel from child-drivers")
path verification requests are much more likely to occur during boot,
resulting in an increased chance of this race condition occurring.

A deferred condition code 1 response indicates that I/O was not started
and should be retried. The current QDIO implementation handles a cc1
response as I/O error, resulting in a failed QDIO setup. This can happen
for example when a path verification request arrives at the same time
as QDIO setup I/O is started.
Fix this by retrying the QDIO setup I/O when a cc1 response is received.
Note that since
commit 2297791c92d0 ("s390/cio: dont unregister subchannel from child-drivers")
commit 5ef1dc40ffa6 ("s390/cio: fix invalid -EBUSY on ccw_device_start")
deferred cc1 responses are much more likely to occur. See the commit
message of the latter for more background information.

Since [1], dma_alloc_coherent() does not accept requests for GFP_COMP
anymore, even on archs that may be able to fulfill this. Functionality that
relied on the receive buffer being a compound page broke at that point:
The SMC-D protocol, that utilizes the ism device driver, passes receive
buffers to the splice processor in a struct splice_pipe_desc with a
single entry list of struct pages. As the buffer is no longer a compound
page, the splice processor now rejects requests to handle more than a
page worth of data.
Replace dma_alloc_coherent() and allocate a buffer with folio_alloc and
create a DMA map for it with dma_map_page(). Since only receive buffers
on ISM devices use DMA, qualify the mapping as FROM_DEVICE.
Since ISM devices are available on arch s390, only, and on that arch all
DMA is coherent, there is no need to introduce and export some kind of
dma_sync_to_cpu() method to be called by the SMC-D protocol layer.
Analogously, replace dma_free_coherent by a two step dma_unmap_page,
then folio_put to free the receive buffer.

The IO subsystem expects a driver to retry a ccw_device_start, when the
subsequent interrupt response block (irb) contains a deferred
condition code 1.
Symptoms before this commit:
On the read channel we always trigger the next read anyhow, so no
different behaviour here.
On the write channel we may experience timeout errors, because the
expected reply will never be received without the retry.
Other callers of qeth_send_control_data() may wrongly assume that the ccw
was successful, which may cause problems later.
Note that since

Tests with hot-plugging crytpo cards on KVM guests with debug
kernel build revealed an use after free for the load field of
the struct zcrypt_card. The reason was an incorrect reference
handling of the zcrypt card object which could lead to a free
of the zcrypt card object while it was still in use.

The fix is simple: Before use of the queue not only the queue object
but also the card object needs to increase it's reference count
with a call to zcrypt_card_get(). Similar after use of the queue
not only the queue but also the card object's reference count is
decreased with zcrypt_card_put().

Once the discipline is associated with the device, deleting the device
takes care of decrementing the module's refcount. Doing it manually on
this error path causes refcount to artificially decrease on each error
while it should just stay the same.

All log messages in dasd.c use the printk variants of pr_*(). They all
add the name of the affected device manually to the log message.
This can be simplified by using the dev_*() variants of printk, which
include the device information and make a separate call to dev_name()
unnecessary.
The KMSG_COMPONENT and the pr_fmt() definition can be dropped. Note that
this removes the "dasd: " prefix from the one pr_info() call in
dasd_init(). However, the log message already provides all relevant
information.

The s390 common I/O layer (CIO) returns an unexpected -EBUSY return code
when drivers try to start I/O while a path-verification (PV) process is
pending. This can lead to failed device initialization attempts with
symptoms like broken network connectivity after boot.
Fix this by replacing the -EBUSY return code with a deferred condition
code 1 reply to make path-verification handling consistent from a
driver's point of view.
The problem can be reproduced semi-regularly using the following process,
while repeating steps 2-3 as necessary (example assumes an OSA device
with bus-IDs 0.0.a000-0.0.a002 on CHPID 0.02):

Background information:
The common I/O layer starts path-verification I/Os when it receives
indications about changes in a device path's availability. This occurs
for example when hardware events indicate a change in channel-path
status, or when a manual operation such as a CHPID vary or configure
operation is performed.
If a driver attempts to start I/O while a PV is running, CIO reports a
successful I/O start (ccw_device_start() return code 0). Then, after
completion of PV, CIO synthesizes an interrupt response that indicates
an asynchronous status condition that prevented the start of the I/O
(deferred condition code 1).
If a PV indication arrives while a device is busy with driver-owned I/O,
PV is delayed until after I/O completion was reported to the driver's
interrupt handler. To ensure that PV can be started eventually, CIO
reports a device busy condition (ccw_device_start() return code -EBUSY)
if a driver tries to start another I/O while PV is pending.
In some cases this -EBUSY return code causes device drivers to consider
a device not operational, resulting in failed device initialization.
Note: The code that introduced the problem was added in 2003. Symptoms
started appearing with the following CIO commit that causes a PV
indication when a device is removed from the cio_ignore list after the
associated parent subchannel device was probed, but before online
processing of the CCW device has started:

During boot, the cio_ignore list is modified by the cio_ignore dracut
module [1] as well as Linux vendor-specific systemd service scripts[2].
When combined, this commit and boot scripts cause a frequent occurrence
of the problem during boot.

Symptom:
In case of a bad cable connection (e.g. dirty optics) a fast sequence of
network DOWN-UP-DOWN-UP could happen. UP triggers recovery of the qeth
interface. In case of a second DOWN while recovery is still ongoing, it
can happen that the IP@ of a Layer3 qeth interface is lost and will not
be recovered by the second UP.
Problem:
When registration of IP addresses with Layer 3 qeth devices fails, (e.g.
because of bad address format) the respective IP address is deleted from
its hash-table in the driver. If registration fails because of a ENETDOWN
condition, the address should stay in the hashtable, so a subsequent
recovery can restore it.
3caa4af834df ("qeth: keep ip-address after LAN_OFFLINE failure")
fixes this for registration failures during normal operation, but not
during recovery.
Solution:
Keep L3-IP address in case of ENETDOWN in qeth_l3_recover_ip(). For
consistency with qeth_l3_add_ip() we also keep it in case of EADDRINUSE,
i.e. for some reason the card already/still has this address registered.

The 'status' attribute for AP queue devices bound to the vfio_ap device
driver displays incorrect status when the mediated device is attached to a
guest, but the queue device is not passed through. In the current
implementation, the status displayed is 'in_use' which is not correct; it
should be 'assigned'. This can happen if one of the queue devices
associated with a given adapter is not bound to the vfio_ap device driver.

The reason no queues for adapter 0x16 are listed in the guest_matrix is
because queue 16.0005 is not bound to the vfio_ap device driver, so no
queue associated with the adapter is passed through to the guest;
therefore, each queue device for adapter 0x16 should display 'assigned'
instead of 'in_use', because those queues are not in use by a guest, but
only assigned to the mediated device.
Let's check the AP configuration for the guest to determine whether a
queue device is passed through before displaying a status of 'in_use'.

When a queue is unbound from the vfio_ap device driver, it is reset to
ensure its crypto data is not leaked when it is bound to another device
driver. If the queue is unbound due to the fact that the adapter or domain
was removed from the host's AP configuration, then attempting to reset it
will fail with response code 01 (APID not valid) getting returned from the
reset command. Let's ensure that the queue is assigned to the host's
configuration before resetting it.

When a queue is unbound from the vfio_ap device driver, if that queue is
assigned to a guest's AP configuration, its associated adapter is removed
because queues are defined to a guest via a matrix of adapters and
domains; so, it is not possible to remove a single queue.
If an adapter is removed from the guest's AP configuration, all associated
queues must be reset to prevent leaking crypto data should any of them be
assigned to a different guest or device driver. The one caveat is that if
the queue is being removed because the adapter or domain has been removed
from the host's AP configuration, then an attempt to reset the queue will
fail with response code 01, AP-queue number not valid; so resetting these
queues should be skipped.

When filtering the adapters from the configuration profile for a guest to
create or update a guest's AP configuration, if the APID of an adapter and
the APQI of a domain identify a queue device that is not bound to the
vfio_ap device driver, the APID of the adapter will be filtered because an
individual APQN can not be filtered due to the fact the APQNs are assigned
to an AP configuration as a matrix of APIDs and APQIs. Consequently, a
guest will not have access to all of the queues associated with the
filtered adapter. If the queues are subsequently made available again to
the guest, they should re-appear in a reset state; so, let's make sure all
queues associated with an adapter unplugged from the guest are reset.
In order to identify the set of queues that need to be reset, let's allow a
vfio_ap_queue object to be simultaneously stored in both a hashtable and a
list: A hashtable used to store all of the queues assigned
to a matrix mdev; and/or, a list used to store a subset of the queues that
need to be reset. For example, when an adapter is hot unplugged from a
guest, all guest queues associated with that adapter must be reset. Since
that may be a subset of those assigned to the matrix mdev, they can be
stored in a list that can be passed to the vfio_ap_mdev_reset_queues
function.

When adapters and/or domains are added to the host's AP configuration, this
may result in multiple queue devices getting created and probed by the
vfio_ap device driver. For each queue device probed, the matrix of adapters
and domains assigned to a matrix mdev will be filtered to update the
guest's APCB. If any adapters or domains get added to or removed from the
APCB, the guest's AP configuration will be dynamically updated (i.e., hot
plug/unplug). To dynamically update the guest's configuration, its VCPUs
must be taken out of SIE for the period of time it takes to make the
update. This is disruptive to the guest's operation and if there are many
queues probed due to a change in the host's AP configuration, this could be
troublesome. The problem is exacerbated by the fact that the
'on_scan_complete' callback also filters the mdev's matrix and updates
the guest's AP configuration.
In order to reduce the potential amount of disruption to the guest that may
result from a change to the host's AP configuration, let's bypass the
filtering of the matrix and updating of the guest's AP configuration in the
probe callback - if due to a host config change - and defer it until the
'on_scan_complete' callback is invoked after the AP bus finishes its device
scan operation. This way the filtering and updating will be performed only
once regardless of the number of queues added.

While filtering the mdev matrix, it doesn't make sense - and will have
unexpected results - to filter an APID from the matrix if the APID or one
of the associated APQIs is not in the host's AP configuration. There are
two reasons for this:
1. An adapter or domain that is not in the host's AP configuration can be
assigned to the matrix; this is known as over-provisioning. Queue
devices, however, are only created for adapters and domains in the
host's AP configuration, so there will be no queues associated with an
over-provisioned adapter or domain to filter.
2. The adapter or domain may have been externally removed from the host's
configuration via an SE or HMC attached to a DPM enabled LPAR. In this
case, the vfio_ap device driver would have been notified by the AP bus
via the on_config_changed callback and the adapter or domain would
have already been filtered.
Since the matrix_mdev->shadow_apcb.apm and matrix_mdev->shadow_apcb.aqm are
copied from the mdev matrix sans the APIDs and APQIs not in the host's AP
configuration, let's loop over those bitmaps instead of those assigned to
the matrix.

The vfio_ap_mdev_filter_matrix function is called whenever a new adapter or
domain is assigned to the mdev. The purpose of the function is to update
the guest's AP configuration by filtering the matrix of adapters and
domains assigned to the mdev. When an adapter or domain is assigned, only
the APQNs associated with the APID of the new adapter or APQI of the new
domain are inspected. If an APQN does not reference a queue device bound to
the vfio_ap device driver, then it's APID will be filtered from the mdev's
matrix when updating the guest's AP configuration.
Inspecting only the APID of the new adapter or APQI of the new domain will
result in passing AP queues through to a guest that are not bound to the
vfio_ap device driver under certain circumstances. Consider the following:
guest's AP configuration (all also assigned to the mdev's matrix):

When domain 4 is re-assigned, since only domain 4 will be inspected, the
APQNs that will be examined will be:

Since both of those APQNs reference queue devices that are bound to the
vfio_ap device driver, nothing will get filtered from the mdev's matrix
when updating the guest's AP configuration. Consequently, queue 16.0005
will get passed through despite not being bound to the driver. This
violates the linux device model requirement that a guest shall only be
given access to devices bound to the device driver facilitating their
pass-through.
To resolve this problem, every adapter and domain assigned to the mdev will
be inspected when filtering the mdev's matrix.

In the vfio_ap_irq_enable function, after the page containing the
notification indicator byte (NIB) is pinned, the function attempts
to register the guest ISC. If registration fails, the function sets the
status response code and returns without unpinning the page containing
the NIB. In order to avoid a memory leak, the NIB should be unpinned before
returning from the vfio_ap_irq_enable function.
