Removed Tx hang detection mechanism from igbvf.
This mechanism has no affect and can cause false alarm message in some cases.

Previously the driver tweaked txqueuelen to avoid false Tx hang reports seen at half duplex.
This had the effect of overriding user set values on link change/reset. Testing shows that
adjusting only the timeout factor is sufficient to prevent Tx hang reports at half duplex.

This change makes it so that reset/interrupt storms can be avoided when
there are mailbox issues. The new behavior is to only allow the device to
trigger mailbox related resets only once every 10 seconds.

This patch removes the skb_dma_map/unmap calls from the igbvf driver due to
the fact that it does not play well with HW IOMMU when combined with
transmitting cloned skbs.

This patch cleans up a number of unused or unneeded feature flags. As a
result of these changes the user should now be able to enable or disable rx
checksumming via ethtool.

The igbvbf driver exposed several unused extrnal references due to the fact
that code was copied from igb and then some functionality was removed.
This changes that so that unused functions are either removed or made
static.

This adds an igbvf driver to handle virtual functions provided by the
igb driver when SR-IOV has been enabled. A virtual function is a
lightweight pci-e function that supports a single queue and shares
resources with the 82576 physical function contained within the igb
driver.
To spawn virtual functions from the igb driver all that is needed is to
enable CONFIG_PCI_IOV and have an 82576 Ethernet adapter on a system that
supports SR-IOV in the BIOS. The virtual functions will appear after the
interface is loaded.
