Use netdev_<level> in place of VELOCITY_PRT.
Use pr_<level> in place of printk(KERN_<LEVEL>.
Miscellanea:
o Add pr_fmt to prefix pr_<level> output with "via-velocity: "
o Remove now unused functions and macros
o Realign some logging lines
o Remove devname where pr_<level> is also used

Unlike most networking drivers using begin() and complete() ethtool_ops
callbacks to resume a device which is down and suspend it again when done,
via-velocity does not use standard refcounted infrastructure but sets
device sleep state directly.
With the introduction of netlink ethtool interface, we may have nested
begin-complete blocks so that inner complete() would put the device back to
sleep for the rest of the outer block.
To avoid rewriting an old and not very actively developed driver, just add
a nesting counter and only perform resume and suspend on the outermost
level.

ifa_list is protected by rcu, yet code doesn't reflect this.
Add the __rcu annotations and fix up all places that are now reported by
sparse.
I've done this in the same commit to not add intermediate patches that
result in new warnings.

Remove the pci_* dma functions and replace with the more generic
versions.
In preparation of adding platform support, a new struct device *dev
is added to struct velocity_info which can be used by both the pci
and platform code.

Improve the clarity of the code in preparation for converting the
dma functions to generic versions, which require a struct device *.
This makes it possible to store a 'struct device *dev' in the
velocity_info structure.
