After discovering that wide use of prefetch on modern CPUs could be a net loss instead of a win, net drivers which were relying on the implicit inclusion of prefetch.h via the list headers showed up in the resulting cleanup fallout. Introduce skb_checksum_none_assert() helper so that we keep this assertion documented in driver sources. Currently the driver assumes that queue IDs start at 0 but that's true only for function 0. To support operation on other functions get the start of the queue ranges from FW and offset accordingly. Commit 1704d74894912b8ecc3e95cecd7bde336a0b1bf2 ("cxgb4vf: small changes to message processing structures/macros") was incomplete and causes cxgb4 to write bad TSO descriptors. Fix that up by reverting the offending part of that commit and adjusting field accesses now that they are one level deeper.
