If the previous thing cat'ing $debugfs/rd left the FIFO full, then
subsequent open could deadlock in rd_write() (because open is blocked,
not giving a chance for read() to consume any data in the FIFO). Also
it is generally a good idea to clear out old data from the FIFO.

For newer devices which deprecate gpu-id and do matching based on
chip-id, we need this information in cmdstream dumps so that the
decoding tools know how to decode them.

related things (submit/retire/recover/fault/etc), so replace
struct_mutex with gpu->lock.

For existing adrenos, there is one or more ringbuffer, depending on
whether preemption is supported. When preemption is supported, each
ringbuffer has it's own priority. A submitqueue (which maps to a
gl context or vk queue in userspace) is mapped to a specific ring-
buffer at creation time, based on the submitqueue's priority.
Each ringbuffer has it's own drm_gpu_scheduler. Each submitqueue
maps to a drm_sched_entity. And each submit maps to a drm_sched_job.

This also converts the special msm_gem_get_vaddr_active() to expect the
lock to already be held. There are two call-sites for this, one already
has the lock held, so it is more straightforward to just open-code the
locking for the other caller.

rd_full should be defined outside the CONFIG_DEBUG_FS region, in order
to be able to link the msm driver even when CONFIG_DEBUG_FS is disabled.

Also log buffers with the DUMP flag set, to ensure we capture all useful
cmdstream in crashdump state with modern mesa.
Otherwise we miss out on the contents of "state object" cmdstream
buffers.

Even if we are not dumping the buffer's contents, it is useful to log
their base address and size. This makes it easier to see when different
gpu pointers point to a single buffer, for example higher mipmap levels
of a single texture.

When IB1 is split into multiple cmd buffers, we'd emit multiple
RD_CMDSTREAM_ADDR per submit. But after this packet is handled
by the cffdump parser, it resets it's known buffers on the next
GPUADDR packet, so subsequent RD_CMDSTREAM_ADDR packets from the
same submit would not find their buffers.
Re-work the loop to snapshot all buffers before RD_CMDSTREAM_ADDR
to avoid this problem.

Drop the deprecated drmP.h header file, and trim msm_drv.h
to the relevant include files.
This resulted in a suprisingly many edits as many files relied
on headers included via msm_drv.h.
But msm_drv.h is not supposed to carry include files it do not need, so
the individual files have to include what extra they needs.

When calling debugfs functions, there is no need to ever check the
return value. The function can work or not, but the code logic should
never do something different based on this.

To lower CPU overhead, future userspace will be switching to pinning
iova and avoiding the use of relocs, and only include cmds table entries
for IB1 level cmdstream (but not IB2 or state-groups).
This leaves the kernel unsure what to dump for rd/hangrd cmdstream
dumping. So add a MSM_SUBMIT_BO_DUMP flag so userspace can indicate
buffers that contain cmdstream (or are otherwise important to dump).

Userspace hasn't used submit cmds with submit_offset != 0 for a while,
but this starts cropping up again with cmdstream sub-buffer-allocation
in libdrm_freedreno.
Doesn't do much good to increment the buf ptr before assigning it.

The [v]snprintf() functions return the size that *would have* been
written into the buffer, rather than the size *actually* written.
Which results in us trying to memcpy() past the end of the stack.
What we really want is [v]scnprintf().

Split into two instances, the existing $debugfs/rd which continues to
dump all submits, and $debugfs/hangrd which will be used to dump just
submits that cause gpu hangs (and eventually faults, but that will
require some iommu framework enhancements).

Prep work for adding a debugfs file that dumps just submits which
trigger hangs/faults. In this case the bo may already be in the
MADV_DONTNEED state, but will be still on the active list (since
the submit hasn't completed yet). So the normal check that the
bo is in the WILLNEED state does not apply. (But of course the bo
should definitely not be in the PURGED state!)

Currently the rd dump avoids any buffers marked as WRITE under
the assumption that the contents are not interesting. While it
is true that the contents are uninteresting we should still print
the iova and size for all buffers so that any listening replay
tools can correctly construct the submission.
Print the header for all buffers but only dump the contents for
buffers marked as READ.

Buffer object specific resources like pages, domains, sg list
need not be protected with struct_mutex. They can be protected
with a buffer object level lock. This simplifies locking and
makes it easier to avoid potential recursive locking scenarios
for SVM involving mmap_sem and struct_mutex. This also removes
unnecessary serialization when creating buffer objects, and also
between buffer object creation and GPU command submission.

Output the upper 32 bits of a 64 bit iova in the RD_CMDSTREAM_ADDR
section while maintaining backwards compatibility for tools that
only understand 32 bit iovas.

drm_debugfs_cleanup() now removes all minor->debugfs_list entries
automatically, so it's not necessary to call
drm_debugfs_remove_files(). Additionally it uses
debugfs_remove_recursive() to clean up the debugfs files, so no need
to do that.

By default, if using $debugfs/.../rd to log cmdstream, only the
cmdstream buffers themselves are logged. But in some cases we want
to capture other buffers in the submit (to see VBO's or shaders).
So add a mod-param knob to control this.

Some, but not all, callers of obj->vmap() would check if return
IS_ERR(). So let's actually return an error if vmap() fails. And fixup

To ease debugging, add debugfs file which can be cat/tail'd to log
submits, along with fence #. If GPU hangs, you can look at 'gpu'
debugfs file to find last completed fence and current register state,
and compare with logged rd file to narrow down the DRAW_INDX which
triggered the GPU hang.
