Enabling gfxoff quirk results in perfectly usable
graphical user interface on HP 705G4 DM with R5 2400G.
Without the quirk, X server is completely unusable as
every few seconds there is gpu reset due to ring gfx timeout.

Pass pointer reference to amdgpu_bo_unref to clear the correct pointer,
otherwise amdgpu_bo_unref clear the local variable, the original pointer

Before this commit, only submits with both a BO_HANDLES chunk and a
'bo_list_handle' would be rejected (by amdgpu_cs_parser_bos).
But if UMD sent multiple BO_HANDLES, what would happen is:
* only the last one would be really used
* all the others would leak memory as amdgpu_cs_p1_bo_handles would
overwrite the previous p->bo_list value
This commit rejects submissions with multiple BO_HANDLES chunks to
match the implementation of the parser.

The struct assertion is failed because sparse cannot parse
`#pragma pack(push, 1)` and `#pragma pack(pop)` correctly.
GCC's output is still 1-byte-aligned. No harm to memory layout.
The error can be filtered out by sparse-diff, but sometimes
multiple lines queezed into one, making the sparse-diff thinks
its a new error. I'm trying to aviod this by fixing errors.

The comment in the vbios structure says:
// = 128 means EDID length is 128 bytes, otherwise the EDID length = ucFakeEDIDLength*128
This fake edid struct has not been used in a long time, so I'm
not sure if there were actually any boards out there with a non-128 byte
EDID, but align the code with the comment.

[Why]
If VF request full GPU access and the request failed,
the VF driver can get stuck accessing registers for an extended period during
the unload of KMS.
[How]
Set no_hw_access flag when VF request for full GPU access fails
This prevents further hardware access attempts, avoiding the prolonged
stuck state.

Why:
Setting IH_RB_WPTR register to 0 will not clear the RB_OVERFLOW bit
if RB_ENABLE is not set.
How to fix:
Set WPTR_OVERFLOW_CLEAR bit after RB_ENABLE bit is set.
The RB_ENABLE bit is required to be set, together with
WPTR_OVERFLOW_ENABLE bit so that setting WPTR_OVERFLOW_CLEAR bit
would clear the RB_OVERFLOW.

We need to take the reset domain lock before flush hdp. We can't put the
lock inside amdgpu_device_flush_hdp itself because it is used during
reset where we already take the write side lock.

For unified queue, DPG pause for encoding is done inside VCN firmware,
so there is no need to pause dpg based on ring type in kernel.
For VCN3 and below, pausing DPG for encoding in kernel is still needed.

Otherwise GPU may access the stale mapping and generate IOMMU
IO_PAGE_FAULT.
Move this to inside p->mutex to prevent multiple threads mapping and
unmapping concurrently race condition.
After kfd_mem_dmaunmap_attachment is removed from unmap_bo_from_gpuvm,
kfd_mem_dmaunmap_attachment is called if failed to map to GPUs, and
before free the mem attachment in case failed to unmap from GPUs.

As we discussed before[1], soft recovery should be
forwarded to userspace, or we can get into a really
bad state where apps will keep submitting hanging
command buffers cascading us to a hard reset.
