Let's use existing ISS encoding for an watchpoint exception i.e ESR_ELx_WNR
This represents an instruction's either writing to or reading from a memory
location during an watchpoint exception. While here this drops non-standard
macro AARCH64_ESR_ACCESS_MASK.

The cpu_suspend_set_dbg_restorer() function is called by the hw_breakpoint
code but defined in another file. Since the declaration is in the
same file as the caller, the compiler warns about the definition
without a prior prototype:
arch/arm64/kernel/suspend.c:35:13: error: no previous prototype for 'cpu_suspend_set_dbg_restorer' [-Werror=missing-prototypes]
Move it into the corresponding header instead to avoid the warning.

Normally we include the full register name in the defines for fields within
registers but this has not been followed for ID registers. In preparation
for automatic generation of defines add the _EL1s into the defines for
ID_AA64DFR0_EL1 to follow the convention. No functional changes.

The naming scheme the architecture uses for the fields in ID_AA64DFR0_EL1
does not align well with kernel conventions, using as it does a lot of
MixedCase in various arrangements. In preparation for automatically
generating the defines for this register rename the defines used to match
what is in the architecture.

For a number of years, UAPI headers have been split from kernel-internal
headers. The latter are never exposed to userspace, and always built
with __KERNEL__ defined.
Most headers under arch/arm64 don't have __KERNEL__ guards, but there
are a few stragglers lying around. To make things more consistent, and
to set a good example going forward, let's remove these redundant
__KERNEL__ guards.
In a couple of cases, a trailing #endif lacked a comment describing its
corresponding #if or #ifdef, so these are fixes up at the same time.
Guards in auto-generated crypto code are left as-is, as these guards are
generated by scripting imported from the upstream openssl project
scripts. Guards in UAPI headers are left as-is, as these can be included
by userspace or the kernel.
There should be no functional change as a result of this patch.

arch_check_bp_in_kernelspace()
We can't pass the breakpoint directly on arch_check_bp_in_kernelspace()
anymore because its architecture internal datas (struct arch_hw_breakpoint)
are not yet filled by the time we call the function, and most
implementation need this backend to be up to date. So arrange the
function to take the probing struct instead.

read_system_reg() can readily be confused with read_sysreg(),
whereas these are really quite different in their meaning.
This patches attempts to reduce the ambiguity be reserving "sysreg"
for the actual system register accessors.
read_system_reg() is instead renamed to read_sanitised_ftr_reg(),
to make it more obvious that the Linux-defined sanitised feature
register cache is being accessed here, not the underlying
architectural system registers.
cpufeature.c's internal __raw_read_system_reg() function is renamed
in line with its actual purpose: a form of read_sysreg() that
indexes on (non-compiletime-constant) encoding rather than symbolic
register name.

ARM64 hardware supports watchpoint at any double word aligned address.
However, it can select any consecutive bytes from offset 0 to 7 from that
base address. For example, if base address is programmed as 0x420030 and
byte select is 0x1C, then access of 0x420032,0x420033 and 0x420034 will
generate a watchpoint exception.
Currently, we do not have such modularity. We can only program byte,
halfword, word and double word access exception from any base address.
This patch adds support to overcome above limitations.

A while back we added {read,write}_sysreg accessors to handle accesses
to system registers, without the usual boilerplate asm volatile,
temporary variable, etc.
This patch makes use of these across arm64 to make code shorter and
clearer. For sequences with a trailing ISB, the existing isb() macro is
also used so that asm blocks can be removed entirely.
A few uses of inline assembly for msr/mrs are left as-is. Those
manipulating sp_el0 for the current thread_info value have special
clobber requiremends.

Even though perf_ops_bp was removed/renamed back in commit
b0a873ebbf87bf38 ("perf: Register PMU implementations"), as part of
v2.6.37, its definition still lives on in some arch headers.
This patch removes the vestigal definition from arm64.

With VHE, we place kernel {watch,break}-points at EL2 to get things
like kgdb and "perf -e mem:..." working.
This requires a bit of repainting in the low-level encore/decode,
but is otherwise pretty simple.

I'm not sure what I was on when I wrote this, but when iterating over
the hardware watchpoint array (hbp_watch_array), our index is off by
ARM_MAX_BRP, so we walk off the end of our thread_struct...
... except, a dodgy condition in the loop means that it never executes
at all (bp cannot be NULL).
This patch fixes the code so that we remove the bp check and use the
correct index for accessing the watchpoint structures.
