The spear13xx version of spear.h is completely different from
the newly combined spear3xx/spear6xx version, but we can never
build ARMv5 and ARMv7 platforms together, so there is no
harm in putting all the contents into a single file and adding
appropriate ifdefs.

This patch moves some global macro definitions to the files where they are used.
Its a step towards removing spear.h completely later on.

viresh.kumar@st.com email-id doesn't exist anymore as I have left the
company. Replace ST's id with viresh.linux@gmail.com.
It also updates .mailmap file to fix address for 'git shortlog'
