Since we're now using addruart to establish the debug mapping, we can remove the io_pg_offst and phys_io members of struct machine_desc. Correct boilerplates after files split. Also shorten them a bit - use standart GPL wording (as per http://lkml.org/lkml/2007/5/1/220) and drop changelog, which only entry about h3800 support and abstracted EGPIOs is just confusing now, as both of these features are gone.
