The patch defines IMX_GPIO_TO_IRQ() in mach/hardware.h and replaces
all the uses of gpio_to_irq() in static initializers with
IMX_GPIO_TO_IRQ().

The patch removes all the uses of cpu_is_mx(). Instead, it utilizes
platform_device_id to distinguish the esdhc differences among SoCs.

mx51_initialize_usb_hw() support introduced a small error. The usbh1 has
pdev->id equal to 1 not 0, so use pdev->id to make things less error-prone and
to fix that.

Most machine files include "devices-imxXX.h" which in turn includes
<mach/devices-common.h>. The latter already includes many headers
that the machine files don't need to include again.
These were found by:
$ grep \#include arch/arm/plat-mxc/include/mach/devices-common.h > tmpfile
$ git grep -l 'devices-imx' arch/arm | xargs grep -f tmpfile -F
(but I kept linux/init.h, linux/kernel.h and linux/platform_device.h)

Note that this relies on stuff currently in mfd's next tree, but this
is also a newer driver. I'm not sure which tree it should go through,
as it's a problem that shows up in next.

The boards are currently using otg_ulpi_create and mxc_ulpi_access_ops,
both are only present if CONFIG_USB_ULPI is set. To remove the need of
ifdefs in the board code introduce a imx_otg_ulpi_create functions
which expands to a static inline function if compiled without ulpi.

This patch declares regulators for the efikamx. Use it also to power off the
efikamx.
Unfortunately, on the efikamx to2 boards, this doesn't work but they allow to
power off by setting GPIO 4 13 to high level instead of powering off through
the mc13892.

The Genesi EFIKA MX and EFIKA Smartbook are sharing a lot of things
so it makes sense to create a common file for both devices and a specific
file for each. No functionnal change except dropping uart 1 & 2.
