MMAP records that occur after the build-id header is parsed do not have
their build-id set even if the filename matches an entry from the
header. Set the build-id on these dsos as long as the MMAP record
doesn't have its own build-id set.

This fixes an issue with off target analysis where the local version of
a dso is loaded rather than one from ~/.debug via a build-id.

The maps code has its own header, move the corresponding C function
definitions to their own C file. In the process tidy and minimize
includes.

Functions purely determine a value from the map and don't need to modify
it. Move functions to C file as they are most commonly used via a

This makes 'perf top' abort in some cases, and the right fix will
involve surgery that is too much to do at this stage, so revert for now
and fix it in the next merge window.

ASan reports a memory leak of nsinfo during the execution of

  # perf test "31: Lookup mmap thread"

The leak is caused by a refcounted variable being replaced without
dropping the refcount.

This patch makes sure that the refcnt of nsinfo is decreased whenever a
refcounted variable is replaced with a new value.

Although 'err' has been initialized to -ENOMEM, but it will be reassigned
by the "err = unwind__prepare_access(...)" statement in the for loop. So
that, the value of 'err' is unknown when map__clone() failed.
