The recent kernel added lost count can be read from either read(2) or
ring buffer data with PERF_SAMPLE_READ. As it's a variable length data
we need to access it according to the format info.

But for perf tools use cases, PERF_FORMAT_ID is always set. So we can
only check PERF_FORMAT_LOST bit to determine the data format.

Add sample_read_value_size() and next_sample_read_value() helpers to
make it a bit easier to access. Use them in all places where it reads
the struct sample_read_value.

Make the cpumap arguments const to make it clearer they are in rather
than out arguments. Make two functions static and remove external
declarations.

When parsing a sample with a sample ID, copy machine_pid and vcpu from
perf_sample_id to perf_sample.

Note, machine_pid will be zero when unused, so only a non-zero value
represents a guest machine. vcpu should be ignored if machine_pid is zero.

Note also, machine_pid is used with events that have come from injecting a
guest perf.data file, however guest events recorded on the host (i.e. using
perf kvm) have the (QEMU) hypervisor process pid to identify them - refer
machines__find_for_cpumode().

Define 2 new flags to represent:

 - when interrupts are disabled (D)
 - when interrupt disabling toggles (t)

This gives 4 combinations:
		no flag, interrupts enabled
t	interrupts were enabled but become disabled
	D	interrupts are disabled
Dt	interrupts were disabled but become enabled

Committer notes:

Those are control flow flags, as per 'tools/perf/Documentation/perf-intel-pt.txt:

&lt;quote&gt;
An interesting field that is not printed by default is 'flags' which can be
displayed as follows:

        perf script --itrace=ibxwpe -F+flags

The flags are "bcrosyiABExgh" which stand for branch, call, return, conditional,
system, asynchronous, interrupt, transaction abort, trace begin, trace end,
in transaction, VM-entry, and VM-exit respectively.

<title>perf tools: Prevent out-of-bounds access to registers</title>

The size of the cache of register values is arch-dependant
(PERF_REGS_MAX). This has the potential of causing an out-of-bounds
access in the function "perf_reg_value" if the local architecture
contains less registers than the one the perf.data file was recorded on.

Since the maximum number of registers is bound by the bitmask "u64
cache_mask", and the size of the cache when running under x86 systems is
64 already, fix the size to 64 and add a range-check to the function
"perf_reg_value" to prevent out-of-bounds access.
