MODULE_SUPPORTED_DEVICE was added in pre-git era and never was
implemented. We can safely remove it, because the kernel has grown
to have many more reliable mechanisms to determine if device is
supported or not.

Use DIV_ROUND_UP() instead of open-coding it. This documents intent
and makes it more clear what is going on for the casual reviewer.

Simplify the code with the new macros for PCM format type iterations.
This fixes the sparse warnings nicely:
