Syzbot has found a deadlock (analyzed by Lance Yang):

1) Task (5749): Holds folio_lock, then tries to acquire i_mmap_rwsem(read lock).
2) Task (5754): Holds i_mmap_rwsem(write lock), then tries to acquire
folio_lock.

migrate_pages()
  -&gt; migrate_hugetlbs()
    -&gt; unmap_and_move_huge_page()     &lt;- Takes folio_lock!
      -&gt; remove_migration_ptes()
        -&gt; __rmap_walk_file()
          -&gt; i_mmap_lock_read()       &lt;- Waits for i_mmap_rwsem(read lock)!

hugetlbfs_fallocate()
  -&gt; hugetlbfs_punch_hole()           &lt;- Takes i_mmap_rwsem(write lock)!
    -&gt; hugetlbfs_zero_partial_page()
     -&gt; filemap_lock_hugetlb_folio()
      -&gt; filemap_lock_folio()
        -&gt; __filemap_get_folio        &lt;- Waits for folio_lock!

The migration path is the one taking locks in the wrong order according to
the documentation at the top of mm/rmap.c.  So expand the scope of the
existing i_mmap_lock to cover the calls to remove_migration_ptes() too.

This is (mostly) how it used to be after commit c0d0381ade79.  That was
removed by 336bf30eb765 for both file &amp; anon hugetlb pages when it should
only have been removed for anon hugetlb pages.

If migration succeeded, we called
folio_migrate_flags()-&gt;mem_cgroup_migrate() to migrate the memcg from the
old to the new folio.  This will set memcg_data of the old folio to 0.

Similarly, if migration failed, memcg_data of the dst folio is left unset.

If we call folio_putback_lru() on such folios (memcg_data == 0), we will
add the folio to be freed to the LRU, making memcg code unhappy.  Running
the hmm selftests:

Likely, nothing else goes wrong: putting the last folio reference will
remove the folio from the LRU again.  So besides memcg complaining, adding
the folio to be freed to the LRU is just an unnecessary step.

The new flow resembles what we have in migrate_folio_move(): add the dst
to the lru, remove migration ptes, unlock and unref dst.

<title>mm/migrate: set swap entry values of THP tail pages properly.</title>

The tail pages in a THP can have swap entry information stored in their
private field. When migrating to a new page, all tail pages of the new
page need to update -&gt;private to avoid future data corruption.

This fix is stable-only, since after commit 07e09c483cbe ("mm/huge_memory:
work on folio-&gt;swap instead of page-&gt;private when splitting folio"),
subpages of a swapcached THP no longer requires the maintenance.

Adding THPs to the swapcache was introduced in commit
38d8b4e6bdc87 ("mm, THP, swap: delay splitting THP during swap out"),
where each subpage of a THP added to the swapcache had its own swapcache
entry and required the -&gt;private field to point to the correct swapcache
entry. Later, when THP migration functionality was implemented in commit
616b8371539a6 ("mm: thp: enable thp migration in generic path"),
it initially did not handle the subpages of swapcached THPs, failing to
update their -&gt;private fields or replace the subpage pointers in the
swapcache. Subsequently, commit e71769ae5260 ("mm: enable thp migration
for shmem thp") addressed the swapcache update aspect. This patch fixes
the update of subpage -&gt;private fields.

We might fail to isolate huge page due to e.g.  the page is under
migration which cleared HPageMigratable.  We should return errno in this
case rather than always return 1 which could confuse the user, i.e.  the
caller might think all of the memory is migrated while the hugetlb page is
left behind.  We make the prototype of isolate_huge_page consistent with
isolate_lru_page as suggested by Huang Ying and rename isolate_huge_page
to isolate_hugetlb as suggested by Muchun to improve the readability.

When clearing a PTE the TLB should be flushed whilst still holding the PTL
to avoid a potential race with madvise/munmap/etc.  For example consider
the following sequence:

  CPU0                          CPU1

  migrate_vma_collect_pmd()
  pte_unmap_unlock()
                                madvise(MADV_DONTNEED)
                                -&gt; zap_pte_range()
                                pte_offset_map_lock()
                                [ PTE not present, TLB not flushed ]
                                pte_unmap_unlock()
                                [ page is still accessible via stale TLB ]
  flush_tlb_range()

In this case the page may still be accessed via the stale TLB entry after
madvise returns.  Fix this by flushing the TLB while holding the PTL.

The D-cache maintenance inside move_to_new_page() only consider one
page, there is still D-cache maintenance issue for tail pages of
compound page (e.g. THP or HugeTLB).

THP migration is only enabled on x86_64, ARM64 and powerpc, while
powerpc and arm64 need to maintain the consistency between I-Cache and
D-Cache, which depends on flush_dcache_page() to maintain the
consistency between I-Cache and D-Cache.

But there is no issues on arm64 and powerpc since they already considers
the compound page cache flushing in their icache flush function.
HugeTLB migration is enabled on arm, arm64, mips, parisc, powerpc,
riscv, s390 and sh, while arm has handled the compound page cache flush
in flush_dcache_page(), but most others do not.

In theory, the issue exists on many architectures.  Fix this by not
using flush_dcache_folio() since it is not backportable.

We notice that hung task happens in a corner but practical scenario when
CONFIG_PREEMPT_NONE is enabled, as follows.

                                                              __migration_entry_wait(tail)

        remove_migration_ptes
            rmap_walk_anon
                cond_resched

Where __migration_entry_wait(tail) is occurred in kernel space, e.g.,
copy_to_user in fstat, which will immediately fault again without
rescheduling, and thus occupy the cpu fully.

When there are too many processes performing __migration_entry_wait on
tail page, remap_page will never be done after cond_resched.

This makes __migration_entry_wait operate on the compound head page,
thus waits for remap_page to complete, whether the THP is split
successfully or roll back.

Note that put_and_wait_on_page_locked helps to drop the page reference
acquired with get_page_unless_zero, as soon as the page is on the wait
queue, before actually waiting.  So splitting the THP is only prevented
for a brief interval.

If the zone device page does not belong to un-addressable device memory,
the variable entry will be uninitialized and lead to indeterminate pte
entry ultimately.  Fix this unexpected case and warn about it.

Currently the kernel is not correctly updating the numa stats for
NR_FILE_PAGES and NR_SHMEM on THP migration.  Fix that.

For NR_FILE_DIRTY and NR_ZONE_WRITE_PENDING, although at the moment
there is no need to handle THP migration as kernel still does not have
write support for file THP but to be more future proof, this patch adds
the THP support for those stats as well.

The kernel updates the per-node NR_FILE_DIRTY stats on page migration
but not the memcg numa stats.

That was not an issue until recently the commit 5f9a4f4a7096 ("mm:
memcontrol: add the missing numa_stat interface for cgroup v2") exposed
numa stats for the memcg.

So fix the file_dirty per-memcg numa stat.
