__folio_split() keeps dereferencing the mapping after the split:
shmem_uncharge(mapping-&gt;host) and remap_page() while the folios are still
frozen/locked, and i_mmap_unlock_read(mapping) at the very end, after the
after-split folios have been unlocked and freed.

Nothing holds an inode reference across that.  The split relies on @folio
-- which the beyond-EOF drop loop never removes, as it starts at
folio_next(folio) -- staying locked and in the page cache to hold off
eviction.  But the unlock loop unlocks @folio before i_mmap_unlock_read()
runs.  If the caller's @lock_at is a tail beyond EOF, as memory_failure()
passes when splitting a poisoned tail of a shmem THP that reaches past
i_size during truncation, it too is gone from the page cache; so once
@folio is unlocked no locked, in-cache folio pins the inode, and a
concurrent final iput() can evict and RCU-free it before
i_mmap_unlock_read() touches i_mmap_rwsem:

Do every mapping dereference while @folio still pins the inode: drop
i_mmap_rwsem right after remap_page(), before the loop that unlocks and
frees the after-split folios, and clear @mapping so the exit path does not
unlock it again.  shmem_uncharge() and remap_page() already run before
that point, so after this nothing past the unlock loop touches the inode
or the mapping.

This is now a rule the split depends on, alongside keeping @folio frozen
until the page cache is updated: no inode or mapping dereference once the
after-split folios start being unlocked.

__split_huge_pmd_locked() updates the file/shmem RSS counter after
dropping the PMD mapping's folio reference.  If folio_put() drops the last
reference, mm_counter_file() can later read freed folio state via
folio_test_swapbacked().

Move the counter update before folio_put().

Commit c010d47f107f ("mm: thp: split huge page to any lower order pages")
introduced an early check on the folio's order via mapping-&gt;flags before
proceeding with the split work.

This check introduced a bug: for shmem folios in the swap cache and
truncated folios, the mapping pointer can be NULL.  Accessing
mapping-&gt;flags in this state leads directly to a NULL pointer dereference.

This commit fixes the issue by moving the check for mapping != NULL before
any attempt to access mapping-&gt;flags.

folio split clears PG_has_hwpoisoned, but the flag should be preserved in
after-split folios containing pages with PG_hwpoisoned flag if the folio
is split to &gt;0 order folios.  Scan all pages in a to-be-split folio to
determine which after-split folios need the flag.

An alternatives is to change PG_has_hwpoisoned to PG_maybe_hwpoisoned to
avoid the scan and set it on all after-split folios, but resulting false
positive has undesirable negative impact.  To remove false positive,
caller of folio_test_has_hwpoisoned() and folio_contain_hwpoisoned_page()
needs to do the scan.  That might be causing a hassle for current and
future callers and more costly than doing the scan in the split code.
More details are discussed in [1].

This issue can be exposed via:
1. splitting a has_hwpoisoned folio to &gt;0 order from debugfs interface;
2. truncating part of a has_hwpoisoned folio in
   truncate_inode_partial_folio().

And later accesses to a hwpoisoned page could be possible due to the
missing has_hwpoisoned folio flag.  This will lead to MCE errors.

Page cache folios from a file system that support large block size (LBS)
can have minimal folio order greater than 0, thus a high order folio might
not be able to be split down to order-0.  Commit e220917fa507 ("mm: split
a folio in minimum folio order chunks") bumps the target order of
split_huge_page*() to the minimum allowed order when splitting a LBS
folio.  This causes confusion for some split_huge_page*() callers like
memory failure handling code, since they expect after-split folios all
have order-0 when split succeeds but in reality get min_order_for_split()
order folios and give warnings.

Fix it by failing a split if the folio cannot be split to the target
order.  Rename try_folio_split() to try_folio_split_to_order() to reflect
the added new_order parameter.  Remove its unused list parameter.

[The test poisons LBS folios, which cannot be split to order-0 folios, and
also tries to poison all memory.  The non split LBS folios take more
memory than the test anticipated, leading to OOM.  The patch fixed the
kernel warning and the test needs some change to avoid OOM.]

<title>mm: prevent poison consumption when splitting THP</title>

When performing memory error injection on a THP (Transparent Huge Page)
mapped to userspace on an x86 server, the kernel panics with the following
trace.  The expected behavior is to terminate the affected process instead
of panicking the kernel, as the x86 Machine Check code can recover from an
in-userspace #MC.

The root cause of this panic is that handling a memory failure triggered
by an in-userspace #MC necessitates splitting the THP.  The splitting
process employs a mechanism, implemented in
try_to_map_unused_to_zeropage(), which reads the pages in the THP to
identify zero-filled pages.  However, reading the pages in the THP results
in a second in-kernel #MC, occurring before the initial memory_failure()
completes, ultimately leading to a kernel panic.  See the kernel panic
call trace on the two #MCs.

[1] Triggered by accessing a hardware-poisoned THP in userspace, which is
    typically recoverable by terminating the affected process.

[2] Call folio_set_has_hwpoisoned() before try_to_split_thp_page().

[3] Pass the RMP_USE_SHARED_ZEROPAGE remap flag to remap_page().

[4] Try to map the unused THP to zeropage.

[5] Re-access pages in the hw-poisoned THP in the kernel.

[6] Triggered in-kernel, leading to a panic kernel.

In Step[2], memory_failure() sets the poisoned flag on the page in the THP
by TestSetPageHWPoison() before calling try_to_split_thp_page().

As suggested by David Hildenbrand, fix this panic by not accessing to the
poisoned page in the THP during zeropage identification, while continuing
to scan unaffected pages in the THP for possible zeropage mapping.  This
prevents a second in-kernel #MC that would cause kernel panic in Step[4].

When both THP and MTE are enabled, splitting a THP and replacing its
zero-filled subpages with the shared zeropage can cause MTE tag mismatch
faults in userspace.

Remapping zero-filled subpages to the shared zeropage is unsafe, as the
zeropage has a fixed tag of zero, which may not match the tag expected by
the userspace pointer.

KSM already avoids this problem by using memcmp_pages(), which on arm64
intentionally reports MTE-tagged pages as non-identical to prevent unsafe
merging.

As suggested by David[1], this patch adopts the same pattern, replacing the
memchr_inv() byte-level check with a call to pages_identical(). This
leverages existing architecture-specific logic to determine if a page is
truly identical to the shared zeropage.

Having both the THP shrinker and KSM rely on pages_identical() makes the
design more future-proof, IMO. Instead of handling quirks in generic code,
we just let the architecture decide what makes two pages identical.

When migrating a THP, concurrent access to the PMD migration entry during
a deferred split scan can lead to an invalid address access, as
illustrated below.  To prevent this invalid access, it is necessary to
check the PMD migration entry and return early.  In this context, there is
no need to use pmd_to_swp_entry and pfn_swap_entry_to_page to verify the
equality of the target folio.  Since the PMD migration entry is locked, it
cannot be served as the target.

Mailing list discussion and explanation from Hugh Dickins: "An anon_vma
lookup points to a location which may contain the folio of interest, but
might instead contain another folio: and weeding out those other folios is
precisely what the "folio != pmd_folio((*pmd)" check (and the "risk of

When an after-split folio is large and needs to be dropped due to EOF,
folio_put_refs(folio, folio_nr_pages(folio)) should be used to drop all
page cache refs.  Otherwise, the folio will not be freed, causing memory
leak.

This leak would happen on a filesystem with blocksize &gt; page_size and a
truncate is performed, where the blocksize makes folios split to &gt;0 order
ones, causing truncated folios not being freed.

When mremap()ing a memory region previously registered with userfaultfd as
write-protected but without UFFD_FEATURE_EVENT_REMAP, an inconsistency in
flag clearing leads to a mismatch between the vma flags (which have
uffd-wp cleared) and the pte/pmd flags (which do not have uffd-wp
cleared).  This mismatch causes a subsequent mprotect(PROT_WRITE) to
trigger a warning in page_table_check_pte_flags() due to setting the pte
to writable while uffd-wp is still set.

Fix this by always explicitly clearing the uffd-wp pte/pmd flags on any
such mremap() so that the values are consistent with the existing clearing
of VM_UFFD_WP.  Be careful to clear the logical flag regardless of its
physical form; a PTE bit, a swap PTE bit, or a PTE marker.  Cover PTE,
huge PMD and hugetlb paths.
