Memory reclaim events are currently difficult to attribute to specific
cgroups, making debugging memory pressure issues challenging.  This patch
adds memory cgroup ID (memcg_id) to key vmscan tracepoints to enable
better correlation and analysis.

For operations not associated with a specific cgroup, the field is
defaulted to 0.

Currently, kswapd_failures is reset in multiple places (kswapd, direct
reclaim, PCP freeing, memory-tiers), but there's no way to trace when and
why it was reset, making it difficult to debug memory reclaim issues.

This patch:

1. Introduce kswapd_clear_hopeless() as a wrapper function to
   centralize kswapd_failures reset logic.

2. Introduce kswapd_test_hopeless() to encapsulate hopeless node
   checks, replacing all open-coded kswapd_failures comparisons.

3. Add kswapd_clear_hopeless_reason enum to distinguish reset sources:
   - KSWAPD_CLEAR_HOPELESS_KSWAPD: reset from kswapd context
   - KSWAPD_CLEAR_HOPELESS_DIRECT: reset from direct reclaim
   - KSWAPD_CLEAR_HOPELESS_PCP: reset from PCP page freeing
   - KSWAPD_CLEAR_HOPELESS_OTHER: reset from other paths

4. Add tracepoints for better observability:
   - mm_vmscan_kswapd_clear_hopeless: traces each reset with reason
   - mm_vmscan_kswapd_reclaim_fail: traces each kswapd reclaim failure

Test results:

reclaim_folio_list uses a dummy reclaim_stat and is not being used.  To
know the memory stat, add a new trace event.  This is useful how how many
pages are not reclaimed or why.

This is an example:

mm_vmscan_reclaim_pages: nid=0 nr_scanned=112 nr_reclaimed=112 nr_dirty=0 nr_writeback=0 nr_congested=0 nr_immediate=0 nr_activate_anon=0 nr_activate_file=0 nr_ref_keep=0 nr_unmap_fail=0

Currently reclaim_folio_list is only called by reclaim_pages, and
reclaim_pages is used by damon and madvise.  In the latest Android,
reclaim_pages is also used by shmem to reclaim all pages in a
address_space.

This isolate_mode_t flag is effectively unused since 89f6c88a6ab4 ("mm:
__isolate_lru_page_prepare() in isolate_migratepages_block()") as
sc-&gt;may_unmap is now checked directly (and only node_reclaim has a mode
that sets it to 0).  The last remaining place is mm_vmscan_lru_isolate
tracepoint for the isolate_mode parameter.  That one was mainly used to
indicate the active/inactive mode, which the trace-vmscan-postprocess.pl
script consumed, but that got silently broken.  After fixing the script by
the previous patch, it does not need the isolate_mode anymore.  So just
remove the parameter and with that the whole ISOLATE_UNMAPPED flag.

We always write out an entire folio at once.  This conversion removes
a few calls to compound_head() and gets the NR_VMSCAN_WRITE statistic
right when writing out a large folio.

Mike Galbraith, Alexey Avramov and Darrick Wong all reported similar
problems due to reclaim throttling for excessive lengths of time.  In
Alexey's case, a memory hog that should go OOM quickly stalls for
several minutes before stalling.  In Mike and Darrick's cases, a small
memcg environment stalled excessively even though the system had enough
memory overall.

Commit 69392a403f49 ("mm/vmscan: throttle reclaim when no progress is
being made") introduced the problem although commit a19594ca4a8b
("mm/vmscan: increase the timeout if page reclaim is not making
progress") made it worse.  Systems at or near an OOM state that cannot
be recovered must reach OOM quickly and memcg should kill tasks if a
memcg is near OOM.

To address this, only stall for the first zone in the zonelist, reduce
the timeout to 1 tick for VMSCAN_THROTTLE_NOPROGRESS and only stall if
the scan control nr_reclaimed is 0, kswapd is still active and there
were excessive pages pending for writeback.  If kswapd has stopped
reclaiming due to excessive failures, do not stall at all so that OOM
triggers relatively quickly.  Similarly, if an LRU is simply congested,
only lightly throttle similar to NOPROGRESS.

Alexey's original case was the most straight forward

	for i in {1..3}; do tail /dev/zero; done

On vanilla 5.16-rc1, this test stalled heavily, after the patch the test
completes in a few seconds similar to 5.15.

Alexey's second test case added watching a youtube video while tail runs
10 times.  On 5.15, playback only jitters slightly, 5.16-rc1 stalls a
lot with lots of frames missing and numerous audio glitches.  With this
patch applies, the video plays similarly to 5.15.

Memcg reclaim throttles on congestion if no reclaim progress is made.
This makes little sense, it might be due to writeback or a host of other
factors.

For !memcg reclaim, it's messy.  Direct reclaim primarily is throttled
in the page allocator if it is failing to make progress.  Kswapd
throttles if too many pages are under writeback and marked for immediate
reclaim.

This patch explicitly throttles if reclaim is failing to make progress.
