Currently the nested interrupt disabling and enabling is represented by
_irqsave() and _irqrestore() APIs, which are relatively unsafe, for
example:

	&lt;interrupts are enabled as beginning&gt;
	spin_lock_irqsave(l1, flag1);
	spin_lock_irqsave(l2, flag2);
	spin_unlock_irqrestore(l1, flags1);
	&lt;l2 is still held but interrupts are enabled&gt;
	// accesses to interrupt-disable protected data will cause races

This is even easier to trigger with guard facilities:

	unsigned long flag2;

	scoped_guard(spin_lock_irqsave, l1) {
		spin_lock_irqsave(l2, flag2);
	}
	// l2 locked but interrupts are enabled.
	spin_unlock_irqrestore(l2, flag2);

(Hand-to-hand locking critical sections are not uncommon for a
fine-grained lock design)

And because of this unsafety, Rust cannot easily wrap the
interrupt-disabling locks in a safe API, which complicates the design.

To resolve this, introduce a new set of interrupt disabling APIs:

*	local_interrupt_disable();
*	local_interrupt_enable();

They work like local_irq_save() and local_irq_restore() except that 1)
the outermost local_interrupt_disable() call saves the interrupt state
into a per-CPU variable, so that the outermost local_interrupt_enable()
can restore the state, and 2) a per-CPU counter is added to record the
nest level of these calls, so that interrupts are not accidentally
enabled inside the outermost critical section.

Also add the corresponding spin_lock primitives: spin_lock_irq_disable()
and spin_unlock_irq_enable(), as a result, code as follows:

	spin_lock_irq_disable(l1);
	spin_lock_irq_disable(l2);
	spin_unlock_irq_enable(l1);
	// Interrupts are still disabled.
	spin_unlock_irq_enable(l2);

doesn't have the issue that interrupts are accidentally enabled.

This also makes the wrapper of interrupt-disabling locks on Rust easier
to design.

As discussed in [1], removing __cond_lock() will improve the readability
of trylock code. Now that Sparse context tracking support has been
removed, we can also remove __cond_lock().

Change existing APIs to either drop __cond_lock() completely, or make
use of the __cond_acquires() function attribute instead.

In particular, spinlock and rwlock implementations required switching
over to inline helpers rather than statement-expressions for their
trylock_* variants.

While Sparse is oblivious to the return value of conditional acquire
functions, Clang's context analysis needs to know the return value
which indicates successful acquisition.

Add the additional argument, and convert existing uses.

Notably, Clang's interpretation of the value merely relates to the use
in a later conditional branch, i.e. 1 ==&gt; context lock acquired in
branch taken if condition non-zero, and 0 ==&gt; context lock acquired in
branch taken if condition is zero. Given the precise value does not
matter, introduce symbolic variants to use instead of either 0 or 1,
which should be more intuitive.

No functional change intended.

Add support for Clang's context analysis for raw_spinlock_t,
spinlock_t, and rwlock. This wholesale conversion is required because
all three of them are interdependent.

To avoid warnings in constructors, the initialization functions mark a
lock as acquired when initialized before guarded variables.

The test verifies that common patterns do not generate false positives.

The check for __LINUX_SPINLOCK_H within rwlock.h (and other files)
detects the direct include of the header file if it is at the very
beginning of the include section.
If it is listed later then chances are high that spinlock.h was already
included (including rwlock.h) and the additional listing of rwlock.h
will not cause any failure.

On PREEMPT_RT this additional rwlock.h will lead to compile failures
since it uses a different rwlock implementation.

Add __LINUX_INSIDE_SPINLOCK_H to spinlock.h and check for this instead
of __LINUX_SPINLOCK_H to detect wrong includes. This will help detect
direct includes of rwlock.h with without PREEMPT_RT enabled.

parisc, ia64 and powerpc32 are the only remaining architectures that
provide custom arch_{spin,read,write}_lock_flags() functions, which are
meant to re-enable interrupts while waiting for a spinlock.

However, none of these can actually run into this codepath, because
it is only called on architectures without CONFIG_GENERIC_LOCKBREAK,
or when CONFIG_DEBUG_LOCK_ALLOC is set without CONFIG_LOCKDEP, and none
of those combinations are possible on the three architectures.

Going back in the git history, it appears that arch/mn10300 may have
been able to run into this code path, but there is a good chance that
it never worked. On the architectures that still exist, it was
already impossible to hit back in 2008 after the introduction of
CONFIG_GENERIC_LOCKBREAK, and possibly earlier.

As this is all dead code, just remove it and the helper functions built
around it. For arch/ia64, the inline asm could be cleaned up, but
it seems safer to leave it untouched.

CONFIG_PREEMPTION is selected by CONFIG_PREEMPT and by
CONFIG_PREEMPT_RT. Both PREEMPT and PREEMPT_RT require the same
functionality which today depends on CONFIG_PREEMPT.

Adjust the comments in the locking code.
