The platform data for the PXAv3 driver allows passing a card
detect GPIO, but this code is not used in the kernel.

In order to not encourage the use of the old global GPIO
numberspace we need to remove this.

Card detect (and write protect) GPIO can easily be added into
the driver using machine descriptor tables instead, and the
descriptor-based (gpiod) variants of the slot GPIO APIs.

struct sdhci_pxa is only used in sdhci_pxa driver itself, so move it
there.

are carried throughout the subsystem. Let's standardise them
to eliminate any confusion.

Tested on OLPC XO-4/MMP3, where the card detection pin for one of
the controllers is a sideband GPIO. The third cell in the cd-gpios
property controls whether the GPIO is active high/active low.

(Also, pass host_caps2 through from platdata to the mmc host.)

As suggested by Arnd, move platform data to include/linux/platform_data
in order to improve build coverage for the driver.
