This commit resolves an issue where modpost section verification fails due
to section mismatches between list_add and its callers.

At present, list_add (and its internal __list_add) are called from both
.text and .init code sections. Since inlining can vary per call site,
list_add can be 4 different states:

  list_add in text with arguments to non-.init.data values
  list_add in init with arguments to static .init.data values
  list_add in init with arguments to non-.init.data values
  list_add in text with arguments to static .init.data values

It is last instance that ends up causing the section mismatch caused by
constant propagation of the address of static libs inside the `dir_add` as
seen below (with the dir_list being defined statically in initramfs.c,
resting in .init.data).

  WARNING: modpost: vmlinux.o: section mismatch in reference: __list_add
  (section: .text.unlikely.) -&gt; dir_list (section: .init.data)

Because of these section matching requirements, semantically, __list_add
and list_add MUST be inlined. This will then ensure callers inside .init
will receive a list_add that exists and refers to only .init data, and
list_add code in .text sections will only refer to non-init data.

This issue manifests predominently in AutoFDO with clang, which is very
hesitant to inline cold functions such as list_add even when marked
`inline`. Marking them as `__always_inline` therefore matches the
existing semantic constraints imposed by modpost's section mismatch
checks.

<title>list: Permit context-unguarded access with list_empty_careful()</title>

With Context Analysis (viz. Clang's Thread Safety Analysis), list_heads
that are __guarded_by(..) require holding the appropriate context lock
when accessing and manipulating them via the list API. Because Clang's
warning diagnostics do not perform inter-procedural analysis, this is
enforced by Clang with -Wthread-safety-pointer in the caller at the call
boundary; a warning is produced when passing a pointer to a guarded
variable without holding the appropriate context locks:

  warning: passing pointer to variable 'list' requires holding [...] [-Wthread-safety-pointer]
           if (list_empty(&amp;ctrl-&gt;list))

An exception is list_empty_careful(), which is like list_empty(), except
that it is permitted to use without holding any context lock (carefully).

Mark list_empty_careful() __context_unsafe, which disables context
analysis within list_empty_careful(), but also suppresses warnings
generated in callers related to its pointer arguments.

Introduce LIST_HEAD_GUARDED(name, lock) to define a struct list_head
annotated with __guarded_by(lock). This provides a convenient shorthand
for defining lock-protected list heads and allows compiler context
analysis to validate accesses to the list against the associated lock.

The new helper also reduces boilerplate and improves consistency across
callers that annotate struct list_head objects with __guarded_by().

This is a preparatory change for subsequent patches that annotate
LIST_HEAD() instances with their protecting lock.

<title>netfs: Fix missing barriers when accessing stream-&gt;subrequests locklessly</title>

<id>urn:sha1:b5782e2d462c028096f922abca46318cec890670</id>
<content type='text'>
The list of subrequests attached to stream-&gt;subrequests is accessed without
locks by netfs_collect_read_results() and netfs_collect_write_results(),
and then they access subreq-&gt;flags without taking a barrier after getting
the subreq pointer from the list. Relatedly, the functions that build the
list don't use any sort of write barrier when constructing the list to make
sure that the NETFS_SREQ_IN_PROGRESS flag is perceived to be set first if
no lock is taken.

Fix this by:

 (1) Add a new list_add_tail_release() function that uses a release barrier
     to set the pointer to the new member of the list.

 (2) Add a new list_first_entry_or_null_acquire() function that uses an
     acquire barrier to read the pointer to the first member in a list (or
     return NULL).

 (3) Use list_add_tail_release() when adding a subreq to -&gt;subrequests.

 (4) Use list_first_entry_or_null_acquire() when initially accessing the
     front of the list (when an item is removed, the pointer to the new
     front iterm is obtained under the same lock).

The list_for_each_rcu() relies on the rcu_dereference() API which is not
provided by the list.h. At the same time list.h is a low-level basic header
that must not have dependencies like RCU, besides the fact of the potential
circular dependencies in some cases. With all that said, move RCU related
API to the rculist.h where it belongs.
