Fix warnings by deleting unused code. The register reads are
kept as it is unknown if there are any hidden side-effects.

This is dead code since 3.15. If there is no plan to use it
further, this can be removed forever.

<title>video: fbdev: nvidia: deprecate pci_get_bus_and_slot()</title>

pci_get_bus_and_slot() is restrictive such that it assumes domain=0 as
where a PCI device is present. This restricts the device drivers to be
reused for other domain numbers.

Getting ready to remove pci_get_bus_and_slot() function in favor of
pci_get_domain_bus_and_slot().

struct nvidia_par has a pointer to struct pci_dev. Use the pci_dev
member to extract the domain information and pass it to
pci_get_domain_bus_and_slot() function.

<title>video: move fbdev to drivers/video/fbdev</title>

The drivers/video directory is a mess. It contains generic video related
files, directories for backlight, console, linux logo, lots of fbdev
device drivers, fbdev framework files.

Make some order into the chaos by creating drivers/video/fbdev
directory, and move all fbdev related files there.

No functionality is changed, although I guess it is possible that some
subtle Makefile build order related issue could be created by this
patch.
