The various struct pci_device_id arrays were initialized mostly by list
expressions. This isn't easily readable if you're not into PCI. Using
named initializers is more explicit and thus easier to parse. Also use
PCI_DEVICE* helper macros to assign .vendor, .device, .subvendor and
.subdevice where appropriate and skip explicit assignments of 0 (which
the compiler takes care of).

The secret plan is to make struct pci_device_id::driver_data an
anonymous union (similar to
https://lore.kernel.org/all/cover.1776579304.git.u.kleine-koenig@baylibre.com/)
and that requires named initializers. But it's also a nice cleanup on
its own.

While touching all these arrays, unify usage of whitespace and comma
in a few drivers.

This change doesn't introduce changes to the compiled pci_device_id
array. Tested on x86 and arm64.

Replace the manual pci_ioremap_wc() call for mapping screen memory with the
device-managed devm_ioremap_wc() variant.

This simplifies the driver's resource management by ensuring the memory is
automatically unmapped when the driver detaches from the device.

The kyro framebuffer driver did not request its PCI memory regions,
which could lead to conflicts with other drivers.  This change
addresses the task "Request memory regions in all fbdev drivers"
from the file Documentation/gpu/todo.rst.

This is addressed by using the managed device functions pcim_enable_device()
and pcim_request_all_regions(). This simplifies the code by making error
handling and driver removal cleanup automatic for these resources.

Set struct fb_ops and with FB_DEFAULT_IOMEM_OPS, fbdev's initializer
for I/O memory. Sets the callbacks to the cfb_ and fb_io_ functions.
Select the correct modules with Kconfig's FB_IOMEM_HELPERS token.

The macro and token set the currently selected values, so there is
no functional change.

The flag FBINFO_DEFAULT is 0 and has no effect, as struct fbinfo.flags
has been allocated to zero by framebuffer_alloc(). So do not set it.

Flags should signal differences from the default values. After cleaning
up all occurrences of FBINFO_DEFAULT, the token will be removed.

Don't populate the const read-only arrays on the stack but instead
make them static const. Use smaller types to use less storage for
the arrays.  Also makes the object code a little smaller.

Update the names of the fb_mem*() helpers to be consistent with their
regular counterparts. Hence, fb_memset() now becomes fb_memset_io(),
fb_memcpy_fromfb() now becomes fb_memcpy_fromio() and fb_memcpy_tofb()
becomes fb_memcpy_toio(). No functional changes.
