vep_alloc_request() wasn't initializing vrequest-&gt;udc, so cancellations
on the FunctionFS AIO path were arriving in vep_dequeue without a valid
UDC reference.

Since vrequest-&gt;udc is never actually properly used anywhere, we opt to
remove it, and update vep_dequeue to obtain a reference to the udc with
ep_to_vudc(), consistent with the other vep_ ops.

AFAICT this bug has existed for ~10 years. Seems that nobody has really
stressed the FunctionFS AIO path on usbip's vudc.

I tested this fix in a QEMU aarch64 guest driving FunctionFS endpoints
via AIO. Before the fix, running `usbip attach` from the host would
cause the guest to oops with the following backtrace:

This patch follows up Zheng Wang's 2023 report of a use-after-free in
vudc_remove(). The original thread stalled on Shuah Khan's request for
runtime testing of the unplug/unbind path. This patch supplies that
testing and keeps Zheng's original fix shape.

In vudc_probe(), v_init_timer() binds udc-&gt;tr_timer.timer to v_timer().
usbip_sockfd_store() starts the timer via v_start_timer()/v_kick_timer().
vudc_remove() can then free the containing struct vudc while the timer is
still pending or executing.

KASAN confirms the race on an unpatched x86_64 QEMU guest with
CONFIG_KASAN=y, CONFIG_USBIP_VUDC=y, CONFIG_USB_ZERO=y, and a tight loop
that repeatedly writes a socket fd to usbip_sockfd, closes the socket
pair, and unbinds/rebinds usbip-vudc.0:

This lands in the timer core rather than v_timer() itself because the
embedded timer_list is being walked after its containing struct vudc has
already been freed. The underlying lifetime bug is the same one Zheng
reported.

With v_stop_timer() called from vudc_remove() and the timer deleted
synchronously, the same harness completed 5000 bind/unbind iterations
with no KASAN report.

Fuzzing uncovered race condition between sysfs code paths in usbip
drivers. Device connect/disconnect code paths initiated through
sysfs interface are prone to races if disconnect happens during
connect and vice versa.

Use sysfs_lock to protect sysfs paths in vudc.

Platform drivers now have the option to have the platform core create
and remove any needed sysfs attribute files.  So take advantage of that
and do not register "by hand" any sysfs files.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner.  So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

This gets rid of the deprecated do_gettimeofday() function in usbip.
The comment above vgadget_get_frame() mentions that it suffers
from issues with the time jumps due to suspend and settimeofday,
so I'm changing it to use ktime_get_ts64() to use monotonic times
that don't have this problem.

I couldn't tell whether we should use CLOCK_MONOTONIC or
CLOCK_MONOTONIC_RAW here, the difference being the exact rate
when correcting for NTP. I picked monotonic time since it doesn't
change the speed to the existing code and should be better
synchronized with other machines we talk to.

vgadget_get_frame returns a frame number from 0 to 2046, which
may require an expensive division operation to wrap at one lower
than the usual number.

I can't see any reason for this, and all other drivers wrap at
a power-of-two number. My best explanation is that it was a simple
typo, so I'm changing the % modulo operator into a cheaper bitmask
that the other drivers use, to make it wrap after 0x7ff rather than
before it.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.
