USB: add SPDX identifiers to all remaining files in drivers/usb/

It's good to have SPDX identifiers in all files to make it easier to audit the kernel tree for correct licenses. Update the drivers/usb/ and include/linux/usb* files with the correct SPDX license identifier based on the license text in the file itself. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text. The core code creates a controller and immediately after that it calls the ->start() callback. This one might drop an error but nobody cares. The same thing happens in the destroy corner: First ->stop() called followed by destroy callback. So why not merge those two into the same function since there is no difference. CONFIG_USB_MUSB_DEBUG option was removed in 5c8a86e usb: musb: drop unneeded musb_debug trickery to cleanup the code from driver specific debug facilities. This patch drops the last references to the musb debug config option, unconditionally enabling all debug code paths, thus letting that code being dropped at compile time if not needed. This patch fixes the following: WARNING: vmlinux.o(.text+0x1e709c): Section mismatch in reference from the funct ion dma_controller_create() to the function .init.text:cppi_controller_start() The function dma_controller_create() references the function __init cppi_controller_start(). This is often because dma_controller_create lacks a __init annotation or the annotation of cppi_controller_start is wrong. This warning is there due to the deficiency in the commit 07a67bbb (usb: musb: Make dma_controller_create __devinit). Since the start() method is only called from musb_init_controller() which is not annotated, drop '__init' annotation from cppi_controller_start() and also cppi_pool_init() since it gets called from that function, to avoid another section mismatch warning...
