Add missing bulk-in endpoint sanity check to prevent uninitialised stack

Unlike the legacy I/O path, scsi-mq preallocates a large array to hold
the scatterlist for each request. This static allocation can consume
substantial amounts of memory on modern controllers which support a
large number of concurrently outstanding requests.

To facilitate a switch to a smaller static allocation combined with a
dynamic allocation for requests that need it, we need to make sure all
SCSI drivers handle chained scatterlists correctly.

Convert remaining drivers that directly dereference the scatterlist
array to using the iterator functions.

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page.  Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

None of these files are actually using any __init type directives
and hence don't need to include &lt;linux/init.h&gt;.  Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

This converts the drivers in drivers/usb/* to use the
module_usb_driver() macro which makes the code smaller and a bit
simpler.

Added bonus is that it removes some unneeded kernel log messages about
drivers loading and/or unloading.
