Fix  using the  bare number to set the 'bDescriptorType' field of the Hub

Fix  using the  bare numbers to set the 'wHubCharacteristics' field of the Hub
Descriptor while the values are #define'd in &lt;linux/usb/ch11.h&gt;.

While at it, avoid duplicate computation when calling DBG().

None of these files are actually using any __init type directives
and hence don't need to include &lt;linux/init.h&gt;.  Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

Individual controller driver has different requirement for wakeup
setting, so move it from core to itself. In order to align with
current etting the default wakeup setting is enabled (except for
chipidea host).

Pass compile test with below commands:
	make O=outout/all allmodconfig
	make -j$CPU_NUM O=outout/all drivers/usb

Drivers should not be putting debug files in /proc/ that is what debugfs
is for, so move the isp1362 driver's debug file to debugfs.
