This implements the V4L2 part of the request support. The main
change is that vb2_qbuf and vb2_prepare_buf now have a new
media_device pointer. This required changes to several drivers
that did not use the vb2_ioctl_qbuf/prepare_buf helper functions.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner.  So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

Make this a proper typed array. Drop the old allocate context code since
that is no longer used.

Note that the memops functions now get a struct device pointer instead of
the struct device ** that was there initially (actually a void pointer to
a struct containing only a struct device pointer).

This code is now a lot cleaner.

Move timestamp from struct vb2_v4l2_buffer to struct vb2_buffer
for common use, and change its type to u64 in order to handling
y2038 problem. This patch also includes all device drivers' changes related to
this restructuring.

The queue_setup callback has a void pointer that is just for V4L2
and is the pointer to the v4l2_format struct that was passed to
VIDIOC_CREATE_BUFS. The idea was that drivers would use the information
from that struct to buffers suitable for the requested format.

After the vb2 split series this pointer is now a void pointer,
which is ugly, and the reality is that all existing drivers will
effectively just look at the sizeimage field of v4l2_format.

To make this more generic the queue_setup callback is changed:
the void pointer is dropped, instead if the *num_planes argument
is 0, then use the current format size, if it is non-zero, then
it contains the number of requested planes and the sizes array
contains the requested sizes. If either is unsupported, then return
-EINVAL, otherwise use the requested size(s).

Remove v4l2 stuff - v4l2_buf, v4l2_plane - from struct vb2_buffer.

Add new member variables - bytesused, length, offset, userptr, fd,
data_offset - to struct vb2_plane in order to cover all information
of v4l2_plane.

Switch this driver over to the V4L2 core locking mechanism in preparation
for switching to unlocked_ioctl. Suggested by Laurent Pinchart.

This patch introduces a new mutex at the struct uvc_video level and
drops the old mutex at the queue level. The new lock is now used for all
ioctl locking and in the release file operation (the driver always has
to take care of locking in file operations, the core only serializes
ioctls).

Note that the mmap and get_unmapped_area file operations no longer take
a lock. Commit f035eb4e976ef5a059e30bc91cfd310ff030a7d3 fixed a AB-BA
deadlock by moving all the locking down into vb2, so the mmap and
get_unmapped_area file operations should no longer do any locking before
calling into vb2.
