USB: make hcd.h public (drivers dependency)

The usbcore headers: hcd.h and hub.h are shared between usbcore, HCDs and a couple of other drivers (e.g. USBIP modules). So, it makes sense to move them into a more public location and to cleanup dependency of those modules on kernel internal headers. Fix the regression created by "set S_DEAD on unlink()..." commit

1) i_flags simply doesn't work for mount/unlink race prevention; we may have many links to file and rm on one of those obviously shouldn't prevent bind on top of another later on. To fix it right way we need to mark _dentry_ as unsuitable for mounting upon; new flag (DCACHE_CANT_MOUNT) is protected by d_flags and i_mutex on the inode in question. Set it (with dont_mount(dentry)) in unlink/rmdir/etc., check (with cant_mount(dentry)) in places in namespace.c that used to check for S_DEAD. Setting S_DEAD is still needed in places where we used to set it (for directories getting killed), since we rely on it for readdir/rmdir race prevention. 2) rename()/mount() protection has another bogosity - we unhash the target before we'd checked that it's not a mountpoint. Fixed. 3) ancient bogosity in pivot_root() - we locked i_mutex on the right directory, but checked S_DEAD on the different (and wrong) one. Noticed and fixed. Signed-off-by: Al Viro

USB: fix char-device disconnect handling

This patch (as1198) fixes a conceptual bug: Somewhere along the line we managed to confuse USB class devices with USB char devices. As a result, the code to send a disconnect signal to userspace would not be built if both CONFIG_USB_DEVICE_CLASS and CONFIG_USB_DEVICEFS were disabled. The usb_fs_classdev_common_remove() routine has been renamed to usbdev_remove() and it is now called whenever any USB device is removed, not just when a class device is unregistered. The notifier registration and unregistration calls are no longer conditionally compiled. And since the common removal code will always be called as part of the char device interface, there's no need to call it again as part of the usbfs interface; thus the invocation of usb_fs_classdev_common_remove() has been taken out of usbfs_remove_device(). Wrap access to task credentials so that they can be separated more easily from the task_struct during the introduction of COW creds. Change most current->(|e|s|fs)[ug]id to current_(|e|s|fs)[ug]id(). Change some task->e?[ug]id to task_e?[ug]id(). In some places it makes more sense to use RCU directly rather than a convenient wrapper; these will be addressed by later patches.
