usb, signal, security: only pass the cred, not the secid, to kill_pid_info_as_cred and security_task_kill

commit d178bc3a708f39cbfefc3fab37032d3f2511b4ec ("user namespace: usb: make usb urbs user namespace aware (v2)") changed kill_pid_info_as_uid to kill_pid_info_as_cred, saving and passing a cred structure instead of uids. Since the secid can be obtained from the cred, drop the secid fields from the usb_dev_state and async structures, and drop the secid argument to kill_pid_info_as_cred. Replace the secid argument to security_task_kill with the cred. Update SELinux, Smack, and AppArmor to use the cred, which avoids the need for Smack and AppArmor to use a secid at all in this hook. Further changes to Smack might still be required to take full advantage of this change, since it should now be possible to perform capability checking based on the supplied cred. The changes to Smack and AppArmor have only been compile-tested. vfs: do bulk POLL* -> EPOLL* replacement

For various bad reasons they aren't the same, and epoll() doesn't actually work quite correctly in some cases due to this on Sparc et al. Pull poll annotations from Al Viro: "This introduces a __bitwise type for POLL### bitmap, and propagates the annotations through the tree. Most of that stuff is as simple as 'make ->poll() instances return __poll_t and do the same to local variables used to hold the future return value'. Some of the obvious brainos found in process are fixed (e.g. POLLIN misspelled as POLL_IN). At that point the amount of sparse warnings is low and most of them are for genuine bugs - e.g. ->poll() instance deciding to return -EINVAL instead of a bitmap. I hadn't touched those in this series - it's large enough as it is. Another problem it has caught was eventpoll() ABI mess; select.c and eventpoll.c assumed that corresponding POLL### and EPOLL### were equal. That's true for some, but not all of them - EPOLL### are arch-independent, but POLL### are not. The last commit in this series separates userland POLL### values from the (now arch-independent) kernel-side ones, converting between them in the few places where they are copied to/from userland. AFAICS, this is the least disruptive fix preserving poll(2) ABI and making epoll() work on all architectures. As it is, it's simply broken on sparc - try to give it EPOLLWRNORM and it will trigger only on what would've triggered EPOLLWRBAND on other architectures. EPOLLWRBAND and EPOLLRDHUP, OTOH, are never triggered at all on sparc. signal: Replace memset(info,...) with clear_siginfo for clarity

USB: remove the URB_NO_FSBR flag

The URB_NO_FSBR flag has never really been used. It was introduced as a potential way for UHCI to minimize PCI bus usage (by not attempting full-speed bulk and control transfers more than once per frame), but the flag was not set by any drivers. There's no point in keeping it around. This patch simplifies the API by removing it. Unfortunately, it does have to be kept as part of the usbfs ABI, but at least we can document in include/uapi/linux/usbdevice_fs.h that it doesn't do anything.
