USB: make single lock for all usb dynamic id lists

There are a number of places where we accidentally pass in a constant structure to later cast it off to a dynamic one, and then attempt to grab a lock on it, which is not a good idea. To help resolve this, move the dynamic id lock out of the dynamic id structure for the driver and into one single lock for all USB dynamic ids. As this lock should never have any real contention (it's only every accessed when a device is added or removed, which is always serialized) there should not be any difference except for some memory savings. Note, this just converts the existing use of the dynamic id lock to the new static lock, there is one place that is accessing the dynamic id list without grabbing the lock, that will be fixed up in a follow-on change. for_each_node_with_property() can iterate through the device_node with a given property name, and we don't need to find the device_node in the while loop. Since of_device_is_available() will return false if the "controller" is NULL and break the while loop, there is no functional change with such conversion. usb: Explicitly include correct DT includes

The DT of_device.h and of_platform.h date back to the separate of_platform_bus_type before it as merged into the regular platform bus. As part of that merge prepping Arm DT support 13 years ago, they "temporarily" include each other. They also include platform_device.h and of.h. As a result, there's a pretty much random mix of those include files used throughout the tree. In order to detangle these headers and replace the implicit includes with struct declarations, users need to explicitly include the correct includes. The USB "maximum-speed" property can now take the SSP signaling rate generation and lane count with these new strings: "super-speed-plus-gen2x2" "super-speed-plus-gen2x1" "super-speed-plus-gen1x2" Introduce usb_get_maximum_ssp_rate() to parse for the corresponding usb_ssp_rate enum. The original usb_get_maximum_speed() will return USB_SPEED_SUPER_PLUS if it matches one of these new strings.
