tty: vt: make consw::con_switch() return a bool

So make this return type a bool explicitly instead of int. The latter might imply that -Eerrors are expected. They are not. And document the hook. This needs a new __fbcon_clear() in the fbcon code to still handle height which might not be 1 when called internally. This may cause info-leak, so to prevent this, it is safest to modify it to initialize the allocated memory space to 0, and it generally does not affect the overall performance of the system. tty: add the option to have a tty reject a new ldisc

They are kind of special, and in particular, the "con_write()" routine violates the "writes cannot sleep" rule that some ldiscs rely on. This avoids the BUG: sleeping function called from invalid context at kernel/printk/printk.c:2659 when N_GSM has been attached to a virtual console, and gsmld_write() calls con_write() while holding a spinlock, and con_write() then tries to get the console lock.
