Replace the existing /* fall through */ comments and its variants with
the new pseudo-keyword macro fallthrough[1]. Also, remove unnecessary
fall-through markings when it is the case.

Use the standard WARN_ON instead.
If a small kernel is desired, WARN_ON can be disabled globally.

Also remove SSB_DEBUG. Besides WARN_ON it only adds a tiny debug check.
Include this check unconditionally.

Also remove SSB_SILENT. Nobody should use it anyway.

I found this core on a BCM4322, a PCI card in the Linksys WRT610N V1.
This core is not used by the driver, this patch just makes ssb show the
correct name.
