scsi: stex: Properly zero out the passthrough command structure

The passthrough structure is declared off of the stack, so it needs to be set to zero before copied back to userspace to prevent any unintentional data leakage. The DEF_SCSI_QCMD() macro passes the addresses of the SCSI host lock and also that of the scsi_done function to the queuecommand_lck() function implementations. Remove the 'scsi_done' argument since its address is now a constant and instead call 'scsi_done' directly from inside the queuecommand_lck() functions. Replace the check for DRIVER_SENSE with a check for scsi_status_is_check_condition(). Audit all callsites to ensure the SAM status is set correctly. For backwards compability move the DRIVER_SENSE definition to sg.h, and update sg, bsg, and scsi_ioctl to set the DRIVER_SENSE driver_status whenever SAM_STAT_CHECK_CONDITION is present. scsi: Replace zero-length array with flexible-array member

The current codebase makes use of the zero-length array language extension to the C90 standard, but the preferred mechanism to declare variable-length types such as these ones is a flexible array member[1][2], introduced in C99: struct foo { int stuff; struct boo array[]; }; By making use of the mechanism above, we will get a compiler warning in case the flexible array does not occur last in the structure, which will help us prevent some kind of undefined behavior bugs from being inadvertently introduced[3] to the codebase from now on. Also, notice that, dynamic memory allocations won't be affected by this change: "Flexible array members have incomplete type, and so the sizeof operator may not be applied. As a quirk of the original implementation of zero-length arrays, sizeof evaluates to zero."[1] This issue was found with the help of Coccinelle.
