Pull SCSI updates from James Bottomley:
 "Usual driver updates (qla2xxx, mpi3mr, mpt3sas, ufs) plus assorted
  cleanups and fixes.

  The biggest core change is the massive code motion in the sd driver to
  remove forward declarations and the most significant change is to
  enumify the queuecommand return"

Firmware populates the negotiated SAS link rate in DevicePage0 during
device discovery. Update mpi3mr to cache this value while initializing
the target device.

When available, the cached link rate is used instead of issuing
additional SAS PHY or expander PHY page reads.  If the DevicePage0 value
is missing or invalid, the driver falls back to the existing PHY-based
mechanism.

The driver was not reading the MAX_REQ_PER_REPLY_QUEUE_LIMIT IOCFacts
flag, so the reply-queue-full handling was never enabled, even on
firmware that supports it. Reading this flag enables the feature and
prevents reply queue overflow.

Allocate segmented trace buffer if firmware advertises the capability in
IOCfacts.

Upon driver load, read the trace buffer size from driver page 1, calculate
the required segments for trace buffer, and allocate segmented buffers.
Each segment is 4096 bytes in size.

While posting driver diagnostic buffer to firmware, advertise that trace
buffer is segmented.

Make driver use the timestamp update interval value provided by firmware in
the driver page 1. If firmware fails to provide non-zero value, then the
driver will fall back to the driver defined macro.
