[ Upstream commit 21bf6fc47a1e45031ba8a7084343b7cfd09ed1d3 ]

Use preferred device_get_match_data() instead of of_match_device() to
get the driver match data. With this, adjust the includes to explicitly
include the correct headers.

The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it as merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

With mt7621 soc_dev_attr fixed to register the soc as a device,
kernel will experience an oops in soc_device_match_attr

This quirk test was introduced in the staging driver in
commit 9445ccb3714c ("staging: mt7621-pci-phy: add quirks for 'E2'
revision using 'soc_device_attribute'"). The staging driver was removed,
and later re-added in commit d87da32372a0 ("phy: ralink: Add PHY driver
for MT7621 PCIe PHY") for kernel 5.11

The way of printing the pointer address for the 'port_base'
address got into compile warnings on some architectures
[-Wpointer-to-int-cast]. Instead of use '%08x' and cast
to an 'unsigned int' just make use of '%px' and avoid the
cast. To avoid not really needed driver verbosity on normal
behaviour change also from 'dev_info' to 'dev_dbg'.

MT7621 SoC clock driver has already mainlined in
'commit 48df7a26f470 ("clk: ralink: add clock driver for mt7621 SoC")'
This allow us to properly use kernel clock apis to get
the clock frequency needed for the phy configuration
instead of use custom architecture code to do the same.
