'struct mdio_device_id' is not modified in these drivers.

Constifying these structures moves some data to a read-only section, so
increase overall security.

On a x86_64, with allmodconfig, as an example:
Before:
======
text data bss dec hex	filename
  27014 12792 0 39806 9b7e	drivers/net/phy/broadcom.o

After:

<title>net: mdiobus: add clause 45 mdiobus accessors</title>

There is a recurring pattern throughout some of the PHY code converting
a devad and regnum to our packed clause 45 representation. Rather than
having this scattered around the code, let's put a common translation
function in mdio.h, and provide some register accessors.

Convert the phylib core, phylink, bcm87xx and cortina to use these.

Since 6e2d85ec0559 ("net: phy: Stop with excessive soft reset")
we don't need genphy_no_soft_reset() any longer. Not setting
callback soft_reset results in a no-op now.

ETHTOOL_LINK_MODE_10000baseT_Full_BIT is set anyway in the supported

And all users of gen10g_config_init use PHY_10GBIT_FEATURES.

cortina_soft_reset() does the same thing as gen10g_soft_reset(), and
cortina_config_aneg() is actually doing what gen10g_config_init() does
for 10G capable PHYs.

This change resolves a new compile-time warning
when built as a loadable module:

WARNING: modpost: missing MODULE_LICENSE() in drivers/net/phy/cortina.o
see include/linux/module.h for more information

This adds the license as "GPL", which matches the header of the file.

MODULE_DESCRIPTION and MODULE_AUTHOR are also added.

Add basic support for Cortina PHY drivers. Support only CS4340 for now.
The phys are not compatible with IEEE 802.3 clause 22/45 registers.

Implement proper read_status support. The generic 10G phy driver causes
bus register access errors.

The driver should be described using the "ethernet-phy-id" device tree
compatible.
