mtd: spear_smi: Use helper function devm_clk_get_enabled()

Since commit 7ef9651e9792 ("clk: Provide new devm_clk helpers for prepared and enabled clocks"), devm_clk_get() and clk_prepare_enable() can now be replaced by devm_clk_get_enabled() when driver enables (and possibly prepares) the clocks for the whole lifetime of the device. Moreover, it is no longer necessary to unprepare and disable the clocks explicitly, so drop the label "err_irq". mtd: Explicitly include correct DT includes

The DT of_device.h and of_platform.h date back to the separate of_platform_bus_type before it was merged into the regular platform bus. As part of that merge prepping Arm DT support 13 years ago, they "temporarily" include each other. They also include platform_device.h and of.h. As a result, there's a pretty much random mix of those include files used throughout the tree. In order to detangle these headers and replace the implicit includes with struct declarations, users need to explicitly include the correct includes. block: replace fmode_t with a block-specific type for block open flags

The only overlap between the block open flags mapped into the fmode_t and other uses of fmode_t are FMODE_READ and FMODE_WRITE. Define a new blk_mode_t instead for use in blkdev_get_by_{dev,path}, ->open and ->ioctl and stop abusing fmode_t. block: use the holder as indication for exclusive opens

The current interface for exclusive opens is rather confusing as it requires both the FMODE_EXCL flag and a holder. Remove the need to pass FMODE_EXCL and just key off the exclusive open off a non-NULL holder. For blkdev_put this requires adding the holder argument, which provides better debug checking that only the holder actually releases the hold, but at the same time allows removing the now superfluous mode argument.
